Job Summary:
The LPN Charge Nurse provides nursing care according to physician orders and in accordance with recognized nursing techniques and procedures, established standards, and facility policies. The LPN Charge Nurse supervises nursing staff on the assigned hall/unit, coordinates resident care, responds to changes in resident condition and emergency situations, and supports a safe, respectful, and compliant care environment.
Duties and Responsibilities:
1. Resident Care and Clinical Nursing (30%)
- Collect and record data for nursing assessments.
- Observe, document, and monitor residents' psychiatric and physical changes and responses to treatment; consult other disciplines when appropriate.
- Initiate and update nursing care plans as resident needs change and evaluate outcomes of nursing care.
- Assess resident care situations, initiate appropriate nursing interventions, and notify the Director of Nursing as appropriate.
- Complete assigned assessments and document resident care accurately in nursing notes.
- Coordinate implementation of physician orders.
- Administer medications, perform treatments, and provide emergency care as ordered or required.
2. Staff Supervision and Unit Leadership (25%)
- Plan, supervise, and evaluate the work of nursing personnel on the assigned unit, providing assistance and guidance as needed.
- Ensure staff under supervision complete assigned duties in a timely manner, according to residents' care plans and facility policies and procedures.
- Communicate direction clearly and concisely and report problems involving ancillary staff to the nursing supervisor.
3. Resident Rights, Communication and Advocacy (15%)
- Provide and direct care with awareness of all Resident Rights, including informed consent, the right to refuse, notification of changes in condition and treatment, privacy and confidentiality, free choice, and freedom from physical or chemical restraints.
- Interact directly with residents, families, and agency representatives on an ongoing basis.
- Participate in resident education and serve as a liaison between residents, physicians, and other members of the interdisciplinary health care team.
- Consult with members of the multidisciplinary health care team at scheduled plan-of-care meetings and other appropriate times.
- Report immediately to the supervisor all instances of suspected abuse or neglect.
4. Unit Operations, Documentation and Professional Responsibilities (15%)
- Maintain accurate records of narcotics, barbiturates, tranquilizers, medications, treatments, and other required nursing documentation.
- Ensure adequate medical and non-medical supplies are available for resident care during the shift and monitor their utilization.
- Participate in Quality Assurance reviews and audits and participate in the development and review of departmental policies and procedures.
5. Compliance and Training (10%)
- Follow all HIPAA privacy regulations, maintaining confidentiality for residents and staff.
- Attend all mandatory in-services, meetings, and training sessions to ensure compliance with facility standards and regulatory updates.
- Demonstrate a commitment to compassionate, customer-focused care to ensure residents' satisfaction and uphold the organization's values.
6. Other Duties as Assigned (5%)
Qualifications:
- Education: Graduate of an accredited school of nursing.
- Licensure: Current LPN license in the state of practice.
- Certification: Maintain current CPR certification.
- Experience: Minimum two (2) years’ experience as an LPN. Experience in a long-term care or skilled nursing facility preferred.
- Skills: Knowledge of nursing and medical practices, regulatory guidelines for long-term care, and experience with staff supervision. Excellent organizational skills, with the ability to prioritize tasks and work independently. Strong leadership and interpersonal skills to effectively communicate with residents, families, and interdisciplinary teams.
Physical Requirements:
- Ability to lift, turn, move and transport residents, and perform physically demanding tasks such as bending, walking, and standing for extended periods.
- Ability to lift up to 50 pounds.
- Capacity to handle stressful situations and respond to emergencies calmly and effectively.
- Work is performed in a healthcare environment, with exposure to infectious diseases, bodily fluids, and potentially hazardous materials.
- Must be able to work various shifts, including weekends, holidays, and overtime as needed.
