About this role

The Community Health Worker will be to act as the liaison and advocate between UC Health patients and "medical homes", community-based healthcare centers, or community providers to advocate for patients through educating, mentoring, coaching and outreach. The primary goal of the Community Healthcare Worker will be to identify patients who are in need of primary care, "medical home" needs, community resources and care coordination to promote basic healthcare access and meet social, physical and developmental needs for a diverse group of patient populations.

- Minimum Required: High School Diploma or GED |
- Ohio Certified Community Health Worker Certification (CCHW) required.
- Recently certified, licensed or graduates within past 12 months.
- Required: 0 - 6 Months equivalent experience; Preferred: 7 - 11 Months equivalent experience.
