About the Program:
Guided by the Jewish tradition of helping all people, Aging Services at JFCS empowers older adults and their families to meet the challenges of aging with dignity, respect, and purpose. Through case management, counseling, education, and resource navigation, the program supports aging adults in maintaining independence, enhancing quality of life, and accessing essential services.
Job Summary:
The Aging Services Case Manager provides strengths-based case management and supportive counseling to older adults and their families. This position plays a key role in helping clients navigate complex systems, build coping skills, and access vital resources to age safely and meaningfully. Services may be provided in a variety of settings, including in-office, in-home, and in the community.
Essential Duties & Responsibilities:
- Provide case management services to clients within the Aging Services program.
- Assume primary responsibility for assessing client and family strengths, challenges, and needs.
- Develop, write, implement, evaluate, and revise treatment and service plans based on ongoing assessments and client/family input.
- Conduct on-going assessment of behavior, mental status, signs and symptoms of mental health disorder, physical and dental health, substance use, and educational, social, legal, financial, housing, developmental and family functioning.
- Refer and link clients and families to appropriate internal and external resources.
- Advocate for client needs and serve as liaison with community agencies/institutions, and support systems to maintain coordination of the treatment process.
- Provide supportive and educational counseling (individual, family, and group) to enhance daily living and coping skills.
- Support clients in building natural support systems to improve stability and reduce isolation.
- Participate in interdisciplinary treatment team meetings and care planning.
- Maintain accurate and timely documentation, including progress notes and assessments, in compliance with agency and funder requirements.
- Complete all required agency and departmental trainings.
- Other duties as assigned.
Requirements
Required Qualifications:
- Bachelor’s degree in Social Work, Psychology, Human Services or related field.
- Minimum of two years’ experience working with the aging population preferred.
- Bilingual (English/Spanish) preferred.
Knowledge, Skills & Abilities:
- Effectively communicate via written, verbal, in person and virtual methods.
- Knowledge and understanding of the characteristics of aging population.
- Knowledge of community resources or ability to learn quickly as well as good organizational, time management and communication skills.
- Ability to work well with community partners and in a team environment is essential.
- Highly motivated, self-directed.
Training Requirements:
- Mandated reporter training (child and adult protection)
- Confidentiality, ethics, and professional boundaries
- Awareness of duty-to-warn and emergency response protocols
- Familiarity with crisis intervention, housing-related services, and trauma-informed care
- JFCS- and accreditation-required onboarding and annual training
Physical Requirements:
- Desk work with computer usage for extended periods.
- Reliable transportation and ability to travel locally.
- Occasional irregular hours and capacity to work under pressure.
- Full range of motion and corrected vision, hearing, and speech (or reasonable accommodations).
