London HART Hub:
The London HART Hub is a low barrier recovery-oriented space that aims to support individuals experiencing homelessness with mental health and addiction needs to improve their health and wellbeing. HART Hubs will support its clients through access to high quality evidence informed care that is respectful, trauma-informed, culturally appropriate and compassionate in nature. It connects individuals with a continuum of care tailored to their needs by providing access to transitional supportive housing spaces where individuals can access a safe place to rest, obtain basic needs, health care, peer supports, employment and income supports, mental health and addiction services, and other services under one location. Individual units allow opportunities for autonomy and independence while accompanying support services ensure basic and personal needs are met while the individual works towards treatment, recovery and wellness.

Addiction and Mental Health Workers within the Hubs program, will perform duties and responsibilities consistent with assisting clients with integrated care planning, system connection and navigation, onsite support with basic needs, mental health and addiction supports.

Withdrawal Management Services (St. Thomas):
The Addiction and Mental Health Worker in the Withdrawal Management Services program will actively support individuals who are experiencing withdrawal from substances in a community setting. Staff will work with existing community services to provide coordinated, collaborative, community-based health and social services from an interdisciplinary, integrated and holistic perspective for people using substances. Providing client-centred care from a harm reduction approach, this direct service position will engage in community development, capacity building and program development with a wide variety of service providers.

W HAT WE'RE OFFERING:

- What you'll be making -  Starting rate for this role is $27.14 per hour (Level 1 – Direct Service), with a wage differential, based on regulation ( $30.01 per hour for Level 1 – Direct Service, Regulated): Successful candidates who are registered with one of the designated professional colleges (e.g. OCSWSSW, CNO, COTO, CRPO, CPSO, or CPO) will receive the wage differential for regulation
- Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8 ($34.41 per hour for non-regulated, and $37.26 per hour for regulated); for internal applicants, grid level placement will occur as per the Collective Agreement
- Premiums of $2.00 per hour will be applied for eligible Evening/Night (8:00pm – 7:00am) and Weekend Shifts.

- When you'll work  - 30 hours biweekly; 10-hour overnight shifts Friday, Saturday and Sunday on alternating weekends; paid meal beak
- Your pension plan  –Immediately eligible for optional enrolment in the Healthcare of Ontario Pension Plan (HOOPP) 
- What your benefits will be –  9% in lieu of benefits

HOW YOU WILL BE MAKING A DIFFERENCE:

HART Hub London:

- Provide intake, screening, assessment and treatment planning
- Support clients on their journey towards physical/mental health improvement
- Ensure clients basic needs are being met (e.g., provision of food, showers, hygiene and a place to rest)
- Provide referrals to other supports and resources to maintain overall wellness
- Assist with the development of care plans, navigating local services, and engaging in proactive crisis intervention
- Provide one-on-one and group counselling sessions with a strengths-based wellness approach
- Ensure client record is updated with changes in circumstances, housing, medications, health conditions, diagnoses, etc.
- Provide navigation assistance, connection to resources, and collaboration with established justice supports for those involved with the criminal justice system (pre-release support, probation or bail)
- Provide accessible options for communication for clients with varying needs and literacy levels (AODA, multiple languages, sign language, audio translation and referral to language classes)
- Assist with transportation needs upon intake, including obtaining travel assistance such as bus passes or other forms of travel, leveraging existing funding support
- Provide integrated care plans that encompass acute and primary care, mental health, addictions, income, transportation and safety
- Provide 24/7, on-site support to help individuals maintain stable housing at HART Hub
- Facilitate ongoing conversations around housing goals to secure suitable housing
- Provide rapid connection to housing stability resources

Withdrawal Management Services:

- Provide client-centred care from a harm reduction approach
- Develop and maintain effective working relationships with a wide range of service providers and organizations
- Acquire and maintain a high level of current knowledge about substance use disorders and trends as well as treatment options
- Facilitate successful service navigation for clients through empowerment and/or advocacy
- Provide education, referrals and support for people using substances and related needs
- Manage indirect services and administrative duties
- Engage in community development, community awareness, education and outreach as designated and appropriate, representing the agency in a professional manner. On occasion, these services may be provided outside of regular office hours
- Attend and actively participate in team meetings and case conferencing meetings
- Maintain awareness of and adherence to policies and protocols
- Support and participate in the quality enhancement activities of the team and agency
- Actively participate in the supervisory process with immediate supervisor
- Provide coverage for other team members as volunteered/directed
- Be supportive of and an active participant in the constant evolution and growth of the agency

WHAT YOU'LL NEED TO APPLY:
  Police Vulnerable Sector Check Requirements
 Successful candidates must have a Police Vulnerable Sector Check completed in the last six months. Interested applicants who reside in the City of London and are in need of a Police Vulnerable Sector Check (PVSC) are encouraged to apply for one as soon as possible as PVSCs in the City of London are currently taking approximately 4-5 weeks to be processed and offers of employment cannot be made without receipt of a PVSC.
 
We require the following qualifications:

- Completed post-secondary education in a mental health or addictions related field  (primary proof of credentials will be required during the hiring process i.e. original degree/diploma or official transcript)
- Valid Driver’s License: G class required
- Transportation of clients in personal vehicle required; two million-dollar ($2 000 000.00) liability insurance required
- Current, satisfactory Police Vulnerable Sector Check (PVSC)
- Demonstrated ability to attend work regularly

We prefer the following qualifications:

- Current certification in Non-Violent Crisis Intervention (NVCI), ASIST Suicide Intervention, and/or Standard First Aid
- Experience working with individuals with addictions or problematic substance use
- Experience working with individuals with no fixed address
- Residential/community housing experience and familiarity with the Residential Tenancies Act
- Experience in crisis intervention/management and crisis resolution
- Experience working in a community-based setting; preferably with an established network of service & support resources
- Experience with, and detailed knowledge of, mild, moderate & serious mental illnesses, usually gained by several years of working with this population
- Clinical work experience ideally in substance abuse assessment and treatment including intake and crisis services
