What You’ll Do

- Provide skilled nursing care based on each client’s individualized care plan
- Assess and monitor clients’ health conditions and clinical needs
- Administer medications and prescribed treatments
- Perform wound care, catheter care, and other skilled nursing procedures within your scope of practice
- Recognize and report changes in the client’s condition
- Coordinate care with physicians, families, and other healthcare team members
- Maintain accurate and timely clinical documentation
- Educate clients and families regarding medications, treatments, and care plans
- Promote the client’s comfort, safety, dignity, and independence
- Provide professional and compassionate care in the client’s home

What We’re Looking For

- Active New Jersey Registered Nurse (RN) License
- 1+ year of nursing experience preferred
- Current CPR/BLS certification
- Strong clinical assessment and communication skills
- Reliable, compassionate, and professional
- Comfortable working independently in a home-care setting
- Valid driver’s license and reliable transportation preferred
- EHR and clinical documentation experience preferred
