Suffolk Family Carers are looking for 4 Care Coordinators .

Are you passionate about helping people get the right support at the right time?

We’re looking for a proactive, organised and compassionate Care Coordinators to join the Care Management Service in Suffolk. This is a rewarding opportunity to coordinate support for individuals and families, working closely with multidisciplinary services to reduce duplication, improve communication and help people access timely, person-centred care.

What you’ll be doing as a Care Coordinator:

- coordinating referrals into the Care Management Service and helping ensure people receive the right support from the right service at the right time
- working with health, social care, voluntary and community partners to ensure joined-up, efficient and person-centred care planning
- maintaining accurate records, signposting to appropriate resources, supporting multidisciplinary meetings and liaising with patients, families and carers by phone, in clinics or during home visits as required.

Who we’re looking for:

- someone with excellent communication and relationship-building skills, able to work positively with patients, carers, professionals and community partners
- a well-organised and resilient team member who can prioritise a varied workload, work under pressure and remain calm when dealing with complex or sensitive situations
- someone confident using digital systems and Microsoft packages, with an awareness of clinical or care systems such as SystmOne being desirable
- a person who shares our commitment to confidentiality, safeguarding, inclusion and improving outcomes for people receiving care and support.

This post attracts an Enhanced DBS check as part of the onboarding process.
