Hourly Rate: £20–£30, depending on relevant experience
Hours: Up to 37.5 hours per week, with the working pattern agreed according to practice requirements
Engagement: Self-employed
Reports to: Designated Clinical Lead and Practice Manager
Professional Registration: Current Nursing and Midwifery Council (NMC) registration

Role Purpose

The General Practice Nurse will provide high-quality, patient-centred nursing care across West PCN. This role is suitable for a registered nurse with established experience working in general practice who can deliver practice nursing services within their competence and agreed scope of practice.

The postholder will work collaboratively with GPs and the wider practice team to support prevention, long-term condition management and patients’ ongoing care needs.

Main Responsibilities

Clinical Care and Long-Term Conditions

- Undertake nursing assessments and develop care plans in partnership with patients.

- Deliver reviews for long-term conditions, such as diabetes, asthma, COPD and cardiovascular disease, where appropriately trained and competent.

- Identify changes in patients’ conditions and escalate concerns to the appropriate clinician.

- Support patients with lifestyle changes, treatment adherence and self-management.

- Contribute to accurate clinical coding and the delivery of relevant Quality and Outcomes Framework (QOF) requirements.

General Practice Nursing Services

- Administer vaccinations and immunisations in accordance with current guidance, appropriate authorisation and demonstrated competence.

- Undertake cervical screening where appropriately trained and authorised.

- Provide wound assessment, dressings and ongoing wound care.

- Undertake blood pressure monitoring, venepuncture and other agreed clinical procedures.

- Provide travel health advice and vaccinations where trained and where the service is offered.

- Provide health promotion and preventive care advice.

- Recognise urgent presentations and arrange appropriate clinical assessment or emergency assistance.

Medicines and Patient Safety

- Administer medicines only within appropriate legal authorisation and local procedures.

- Maintain safe vaccine storage, cold-chain monitoring and stock management.

- Provide patients with clear information about treatment, possible adverse effects and follow-up.

- Prescribe only where appropriately qualified, registered and authorised.

Multidisciplinary Working

- Work collaboratively with GPs, pharmacists, healthcare assistants and other practice colleagues.

- Attend relevant clinical meetings and contribute to coordinated patient care.

- Refer patients to appropriate services and communicate follow-up requirements clearly.

- Support colleagues and share knowledge within the limits of the role.

Clinical Governance and Professional Responsibilities

- Maintain accurate, timely clinical records using practice electronic systems.

- Practise in accordance with the NMC Code and maintain current NMC registration.

- Recognise and report safeguarding concerns in line with local procedures.

- Follow confidentiality, consent, information governance, infection prevention and health and safety requirements.

- Report incidents and near misses and contribute to clinical audit and quality improvement.

- Maintain relevant training, continuing professional development and evidence required for NMC revalidation.

- Work within personal competence and seek clinical advice when required.

Person Specification

Essential Criteria

- Current NMC registration as a Registered Nurse.

- Established experience working as a General Practice Nurse in a UK general practice setting.

- Demonstrated competence in delivering core practice nursing services.

- Relevant training and competence for the clinical duties agreed for the role.

- Experience assessing patients and contributing to long-term condition management.

- Understanding of vaccination safety, infection prevention and safeguarding.

- Ability to recognise clinical deterioration and escalate concerns appropriately.

- Strong communication skills and the ability to provide respectful, patient-centred care.

- Ability to work effectively within a multidisciplinary team.

- Accurate clinical documentation and confidence using electronic patient records.

- Evidence of continuing professional development.

- Right to work in the UK without requiring sponsorship from K&W Healthcare.

- Ability to work across designated West PCN practices as required.

Desirable Criteria

- Post-registration qualifications in diabetes, asthma, COPD or other relevant clinical areas.

- Current cervical screening training and authorisation.

- Experience using EMIS Web or SystmOne.

- Knowledge of QOF and general practice clinical coding.

- Independent prescribing qualification with the appropriate NMC annotation.

- Experience in travel health, clinical audit, service improvement or mentoring.

Engagement Requirements

Engagement is subject to satisfactory references, verification of NMC registration and right to work, appropriate DBS checks, occupational health requirements and confirmation of suitable professional indemnity arrangements.

The scope of services, agreed hours, hourly rate and other terms will be set out in the self-employed services agreement.
