About this role
The MAP Community Health Worker for Medicaid Access Preservation (MAP), under the supervision of the Program Manager of Health Insurance Services, will work closely with the Health Insurance Services team to assist Sam Rodgers patients and community members with Medicaid coverage to help them comply with the Community Engagement (work) requirement; or request an exemption for medical frailty/special medical needs; and renew their Medicaid coverage. The Community Health Worker will help patients and community members who are refugees or asylees update their immigration or citizenship status to the MO Family Support Division (FSD) to maintain Medicaid coverage or help them access healthcare services at Sam Rodgers with our sliding scale discount or other public and private charity care programs. The Community Health Worker will also coordinate patient referrals for health insurance services among various clinic staff and departments.
Qualifications
- Community Health Worker certification or relevant Medicaid program experience and able to get a community health worker certification within 1 year of hire date.
- Must be a high school graduate, GED certified equivalent, and have a minimum of an associate degree or at least two years of work experience in a healthcare organization; community outreach experience; or an understanding of the MOHealthNet/CHIP or KanCare program.
Knowledge of the Medicaid application and renewal process, and reporting household changes to the MO Family Support Division.
- Regularly use and maintain access to the MO FSD IT systems (EMOMED, MEDES and FAMIS) to look up patient and consumer information to help applicants apply or renew their Medicaid coverage.
- Maintain strict confidentiality of all applicants personal information in keeping with HIPAA privacy regulations.
- Must demonstrate an understanding of the socio-economic and health care problems of diverse low-income groups in a large metropolitan area.
Explores alternatives and determines the best solution when trying to resolve customer service concerns.
- Must demonstrate tact, patience, and good communication skills in dealing with patients, Health Center staff, the public, external agencies, and organizations.
- Capacity to help consumers increase their health literacy; understand their Medicaid plans benefits; and how to use their health insurance to access the healthcare system.
- Must be able to type; computer literate; and able to generate reports and prepare presentations.
- Able to communicate effectively orally and in writing.
- Experience using Microsoft Office software products.
- Ability to travel and work at various Sam Rodgers clinics or at off-site locations throughout our service area to achieve program goals.
- Participate in evening and/or weekend community events as needed.
- Maintain a valid drivers license, reliable transportation, and proof of insurance.
Bilingual (Spanish, Somali or another second language) is a plus.
