Social Services Director

Spokane Valley Health and Rehab is seeking a compassionate and resident-focused Social Services Manager to support the emotional, social, and practical needs of our residents. This role serves as a resident advocate, helping protect resident rights, coordinate resources, support care planning, and promote the highest quality of life.
Key Responsibilities
- Conduct psychosocial assessments and develop person-centered interventions.
- Advocate for residents and support resident rights.
- Maintain social services documentation and records.
- Assist residents and families with accessing community resources and services.
- Serve as a liaison between residents, families, caregivers, and community agencies.
- Participate in care planning meetings and interdisciplinary team discussions.
- Support behavior management interventions that promote resident well-being and safety.
- Assist with discharge planning and transitions of care.
- Collaborate with nursing, therapy, and other departments to support resident outcomes.
- Help address social, emotional, and environmental factors affecting resident health.
Qualifications
- Experience in long-term care, skilled nursing, healthcare, or social services preferred.
- Strong communication, organization, and relationship-building skills.
- Ability to work effectively with residents, families, and interdisciplinary teams.
- Commitment to providing compassionate, resident-centered care.

Requirements
Education
- Preferred: Bachelor’s or Master’s degree in social work.
- Must have experience in Social Services. Preferred experience in Skilled Nursing Facility.
Licenses/Certification
- LCSW, LSW, or MSW license preferred.
- Must have a valid driver’s license.
Experience
- Experience in post-acute/ skilled nursing preferred.

Starting at $38.00 depending on experience
