OVERVIEW : Under the supervision of the Family Facilitator Supervisor, the Family Facilitator performs assessments services and development service plans for youth and families admitted into the Full Service Partnership Wraparound program. May perform services related to family stabilization services as well. Knowledge of HFW, FSP, and family crisis and stabilization models is essential.

1. (75%) Direct Services

- Contributes to assessment, development, implementation, and evaluation of individual wraparound plans.

- Coordinate, schedule, and facilitate child and family team meetings (CFT) for development of individualized wraparound plans.

- Provide oversight of and ensure timely access to CFT meetings.

- Provide and attend Emergency CFT meetings in response to serious mental health incidents.

- Coordinate services for families based on their level of need identified by the CFT, including, but not limited to, referrals for psychiatric services, advocacy, therapy, alcohol and drug treatment.

- Assume responsibility for the applicable treatment/service model and implementation of services/coordination of resources for children and families.

- Monitor and support the delivery of services by reassessing and revising individual plans as needed, with LPHA feedback, ensuring that families have access to resources, and maintaining effective relationships with child and family team members.

- Communicate symptom information to LPHA to assure appropriate diagnosis is assigned in EHR.

- Provide direct services (e.g., short-term counseling, problem-solving) to youth and/or family when necessary.

- Provide crisis management to youth and families through development of safety/crisis plans as well as administration of such plans.

- Participate in the Wrap crisis on-call system.

- Complete initial and annual clinical bundles using EHR—this includes assessment (Cal-AIMs, client plan, CANS, etc.)

- Write progress notes for each client/family session within required deadlines.

- Complete all documents for the program, including but not limited to, CFT minutes, family safety plan, family budget, etc., in a timely manner.

- Assume responsibility for ongoing child and family team development.

- Assist in the ongoing development and evaluation of the service program.

2. (25%) Other

- Serve as a role model and advocate for children and families involved in program.

- Monitor and ensure effective linkages with community and agency service providers.

- Participate in training activities as directed.

- Establish and maintain positive and effective working relationships with internal staff and external resources.

- Adhere to policies, systems and standards of the Sacramento Childrens Home and all applicable programs, agencies and contracts.

- Participate in supervision, staff meetings, process reviews and other assigned meetings.

- Perform other duties as assigned.

LOCATION : 2750 Sutterville Rd., Sacramento, CA 95820

SCHEDULE: Monday – Friday, 9:00am – 5:30pm. Schedule may be modified to include evenings and/or weekend based on business needs.

SALARY : $25.94 - $29.18 per hour, to start

Qualifications

Driving is an essential function of this job, and as such, employees in this position will be required to furnish their own vehicle to be used in the course of employment. A valid CA drivers license and minimum insurance coverage is also required.

Must be at least 21 years of age.

Education and Experience:

Must possess a Masters Degree and 2 years of full-time/equivalent (FTE) direct care experience in a mental health setting or a Bachelors degree and 4 FTE direct care experience in a mental health setting or an Associate Arts Degree and six years of FTE direct care experience in a mental health setting. At least two of the six years must be post-AA degree experience in a mental health setting.

At least 2 years experience working with seriously disturbed children, adolescents, and families, and experience in engaging parents as partners and working successfully with families from diverse ethnic and cultural groups is preferred.

Previous experience with EPSDT billing and documentation is highly desired.
