Job Summary and Responsibilities

As our RN Transition Fellow, you will provide professional nursing care to patients of all ages, adhering to individual needs, physician orders, Hospital policy, and nursing standards. You will also assume responsibility for directing and coordinating care team members, focusing on individualized quality patient care consistent with organizational values.Every day, you will collaborate with the healthcare team to implement comprehensive patient care plans and participate in direct patient care activities, including assessment, medication administration, documentation, and patient education across diverse patient populations.To be successful in this vital role, you will demonstrate an eagerness to learn and apply new knowledge, showing initiative in developing increasing skills and confidence. You will exhibit strong foundational nursing knowledge, critical thinking, and excellent communication skills.

- Adheres to established Hospital/departmental policies and procedures, objectives, Quality Assessment and Improvement Program, legal, safety, environmental, infection control standards, Nurse Practices Act and Patient's Bill of Rights.

- Applies clinical knowledge and expertise for coordinating patient care by regularly planning and evaluating the care of patients.

- Establishes and maintains professional and collaborative relationships with other staff members, physicians, patients, patient family members, and the public as appropriate, while assuring confidentiality of all patient information.

- Demonstrates effective nursing judgement and a working knowledge of the nursing process.

- Provides guidance to interdisciplinary team members by supervising nursing interventions and assisting with related tasks.

- Provides leadership by establishing priorities and coordinating nursing care.

This position is represented by CNA and is covered by the terms and conditions of the applicable collective bargaining agreement

Job Requirements

Required

- Graduate of Accredited RN school of Nursing Minimum 2 years of RN experience in an acute care setting

- Registered Nurse: CA, upon hire

- AHA Basic Life Support / Advanced Cardiac Life Support

- Fire Safety within 90 days

Preferred

- Bachelors Of Nursing, upon hire or

- Masters Of Nursing, upon hire and

- 4 years RN experience
