JOB DESCRIPTION

Title: Singles Case Manager

Department: Crossroads Case Management Services

Reports to: Case Management Services Manager

FLSA Status: Non-exempt

Directly Leads: Not Applicable

Salary: 55K – 60K

SUMMARY

The Singles Case Manager functions as an integral member of the Crossroads Shelter case management team and provides supportive services to homeless singles individuals who are living in the shelter facility. This position acts as an advocate and assists with finding permanent, affordable housing and employment. The Singles Case Manager is responsible for assessing client housing needs, providing support in researching low-income and other housing resources, assisting in coordinating a search for permanent housing, and maintaining relationships with housing property managers, owners and organizations to facilitate the placement of homeless singles in stable housing situations and mediate between landlords and tenants if needed. The Case Manager is responsible for providing tools for basic living, via basic life skills and referrals to training services as appropriate. The Singles Case Manager follows a Housing First model and is experienced in building a client centered culture to services.

Service Delivery

- Meet weekly with homeless individuals to assess/prioritize individual goals.

- Provide information and referrals to increase residents ability to achieve goals.

- Monitor the progress and activities of residents and adjust goals as needed.

- Assist residents to access a continuum of support services that will enable residents to:

Secure permanent housing opportunities

- Develop employment or other income eligibilities

- Provide budgeting skills

- School enrollment (if applicable)

- Make connections to medical, mental health, and substance abuse resources as applicable

- Refer to educational workshops, i.e. weekly life skills

Qualifications

QUALIFICATIONS

- Applicant must have three to five years of case management experience.

- Able to motivate homeless individuals to realize their full potential. Motivational Interviewing experience/training preferred

- Housing First exposure preferred

- Excellent verbal and written communication skills, effective organizational and interpersonal skills

- Knowledge of community resources and service providers who work with homeless people

- Able to demonstrate flexibility and possess the ability to respond to the needs of homeless individuals and families

- Able to assist homeless women and men who might need resources for their children

- Self-directed, yet must be willing to request guidance or assistance.

- Thorough, well-organized and competent in managing detailed case files.

- Skilled in conflict resolution

EDUCATION and/or EXPERIENCE

- Bachelors degree or comparable work experiences

- Comfortable and willing to work in an inner-city environment

- Comfortable in a multi-cultural setting

- Appropriate professional and personal boundaries with high integrity Knowledgeable about the dynamics of homelessness, substance abuse, mental health, domestic violence, and parenting

LANGUAGE SKILLS: English, second language, especially Spanish or American Sign Language, a plus

MATHEMATICAL SKILLS: College level

REASONING ABILITY: Must have the ability to utilize good judgment, think logically, and make sound decisions. Ability to outline realistic service plans.

COMPUTER SKILLS: Experience working with computers and successful utilization of Word, Excel and Outlook. Must possess knowledge of the internet and be able to retrieve resources and email. Experienced with databases that track client demographics and outcomes.

CERTIFICATE, LICENSES, REGISTRATIONS: California Drivers license and insurance

PHYSICAL REQUIREMENTS: Must dress neatly and professionally when reporting to work. This position requires an annual tuberculosis test.

WORK ENVIRONMENT: Ability to work in a diverse and stressful environment with residents with multiple issues.
