Are you a passionate leader who thrives in a fast-paced environment and is committed to making a difference every day? As a Senior Access and Assessment Officer, you will play a pivotal role in leading the front door to adult health and social care services in East Sussex.
Working at the heart of Health and Social Care Connect, you will support and develop a skilled team, drive high-quality decision-making, and help ensure adults and carers receive the right support at the right time.
Health and Social Care Connect (HSCC) is the point of access into adult community health and social care services in East Sussex. The service provides residents with information and advice and manages professional referrals into community health and social care services. HSCC also provides assessment and safeguarding triage functions for adults across the county.
We are looking for someone who has a working knowledge of adult social care and some supervisory or management experience to join our team.
As part of your role, you will be prioritising and triaging referrals coming in, providing information and support to staff and liaising with our partners both internally and externally.
The right person for this role will be enthusiastic, have demonstrable working knowledge of adult social care or community health services, be motivated and able to deal with competing priorities in a dynamic and fast paced environment.
You can expect to receive regular supervision and comprehensive training and continuing professional development.
You will also have line management responsibility for staff who provide a 24/7 service. Whilst you will not be required to work shifts you will be required to participate in the on-call rota out of hours to provide management support and advice.
We are looking for candidates who have:
- Experience of supervising, coaching or managing staff within a health, social care or customer-facing service.
- Knowledge and understanding of adult social care assessment processes, safeguarding responsibilities, and the importance of person-centred practice.
- Ability to make informed decisions in a fast-paced environment, including managing complex or high-risk situations and escalating concerns appropriately.
- Experience of working effectively with a range of partner organisations, including health, social care, voluntary sector and community services.
- Strong communication and organisational skills, with the ability to coordinate workloads, facilitate meetings, manage competing priorities and support service delivery.
