This position is funded by a five-year grant and is expected to continue for the duration of the grant funding, up to five years. We are seeking a candidate who is interested in remaining in the position for that anticipated period. Position Description:

Leads strategic provider engagement and practice transformation efforts supporting Tennessee's Rural Health Transformation initiatives. Partners with participating providers and internal stakeholders to improve provider performance, access to care, quality outcomes, and program success.

Primary Job Responsibilities:

- Develops provider engagement and practice transformation strategies aligned with Rural Health Transformation goals, provider performance, access measures, and community needs.
- Provides strategic support to Provider Specialists by preparing meeting materials, presentations, scorecards, talking points, educational resources, and follow-up action plans.
- Analyzes provider data, operational trends, and performance measures to identify performance gaps and translate findings into provider-specific recommendations, improvement plans, dashboards, and reports that establish accountability and track results.
- Advises Provider Specialists and providers on difficult conversations, escalations, operational barriers, and improvement opportunities affecting patient care, program participation, and provider performance.
- Develops best-practice frameworks, workflow recommendations, tools, templates, and consistent messaging that support effective provider engagement and sustainable practice transformation.
- Partners with providers and Provider Specialists to establish measurable goals, monitor progress, and support adoption of value-based care, population health, and quality improvement strategies.
- Develops and delivers provider education through workgroups, webinars, training sessions, and collaborative learning opportunities focused on program requirements, quality measures, performance reports, and operational processes.
- Promotes providers’ use of improvement resources, community support, and healthcare transformation opportunities to advance patient outcomes and population health goals.
- Collaborates with Network Management, Quality Improvement, Population Health, Operations, Compliance, Clinical, and other internal teams to resolve provider issues and remove barriers to participation.
- Communicates provider feedback, market intelligence, opportunities, and risks to leadership to inform engagement priorities, strategic decisions, and Rural Health Transformation initiatives.

Perform other related assigned duties as necessary to complete the Primary Job Responsibilities as described above.

Minimum Requirements:

Position requires a bachelor’s degree in healthcare administration, business administration, public health, communications, or a related field, and five years of experience in provider relations, network management, healthcare operations, or account management. Experience with managed care or Medicaid programs and value-based care, population health, or healthcare transformation initiatives is preferred. Will accept any suitable combination of education, training or experience.

Position requires knowledge of practice operations, workflow optimization, quality improvement, practice transformation, dental insurance, and payer operations; understanding of value-based care, dental access, and provider onboarding, recruitment, and retention; strong provider-facing communication, presentation, facilitation, relationship-building, influencing, and stakeholder management skills; ability to develop consultative provider engagement and performance improvement strategies, prepare presentations, scorecards, educational materials, and action plans, and translate data into actionable recommendations; strong analytical, problem-solving, project management, and technology skills, including proficiency with Microsoft Office Suite, reporting tools, and provider management systems.

Position also requires the ability to travel throughout Tennessee to conduct provider visits, meetings, education sessions, and Rural Health Transformation activities.
