Job Description:
POsition Purpose summary:
The Case Worker provides compassionate, participant-centred case management services to individuals and families accessing The Salvation Army's community programs. The Case Worker builds supportive relationships with participants and assists them in identifying goals, addressing barriers, accessing community resources, and developing practical plans for greater stability and independence.
The Case Worker works collaboratively with Salvation Army staff, volunteers, spiritual care representatives, and community partners to provide holistic support to participants while maintaining confidentiality, professional boundaries, and accurate case documentation.
KEY Accountabilities:
Case Management
- Complete participant intake, assessment, goal setting, action planning, transition, and follow-up.
- Develop and maintain an assigned caseload.
- Work collaboratively with participants to identify strengths, needs, barriers, and personal goals.
- Develop individualized action plans and regularly review progress with participants.
- Provide ongoing casework and support in accordance with Salvation Army policies and program requirements.
- Build trusting, respectful, and non-judgmental relationships with participants.
- Provide information, referrals, advocacy, and assistance accessing appropriate community services.
- Accompany or support participants when appropriate in connecting with community resources.
- Conduct meetings with participants in person, by telephone/video, and when appropriate in community or home settings.
- Respond appropriately to crisis situations and assist participants in navigating complex social-service systems.
Community Resources & Partnerships
- Develop and maintain strong working relationships with community agencies and service providers.
- Connect participants with housing, financial, employment, health, mental-health, addictions, family, and other community resources as appropriate.
- Attend community and agency meetings as required.
- Represent The Salvation Army professionally when working with community partners.
- Work collaboratively with Salvation Army staff, volunteers, and ministry teams to provide coordinated support.
Administration & Documentation
- Maintain accurate, confidential, and up-to-date participant files.
- Complete detailed case notes documenting participant contacts, services, referrals, and progress.
- Maintain required program records, statistics, and reports.
- Collect information required for program evaluation and outcome measurement.
- Follow all applicable Salvation Army policies, procedures, confidentiality requirements, and legislation.
- Participate in staff meetings, case consultations, training, and professional development.
Team Collaboration
- Work closely with Community Services staff and other Salvation Army personnel to provide holistic support.
- Participate in Pathway of Hope team meetings and consultations.
- Encourage appropriate involvement of volunteers and community members in supporting participants.
- Support the integration of community services with the broader mission of The Salvation Army.
Working Conditions:
- Primarily office and community-based work.
- Regular interaction with individuals and families experiencing a wide range of social and economic challenges.
- Local travel between Salvation Army locations and community appointments may be required.
- Some evening and weekend work may occasionally be required.
- Ability to lift and move up to approximately 20 lbs.
- The position requires the ability to work respectfully and professionally in a variety of community settings.

Education and Experience Qualifications:
Education, Qualifications and Certifications:
- Completion of a two-year post-secondary diploma in a human-services field, such as Social Services Worker.
- A Bachelor's degree in Social Work, Social Services, or a related field is considered an asset.
- Equivalent combinations of education and relevant experience may be considered.
Experience and Skilled Knowledge Requirements
- Previous experience in case management, social services, community services, or a related field is preferred.
- Experience working with individuals and families experiencing complex social or economic challenges is an asset.
- Current First Aid/CPR certification, or willingness to obtain.
- Non-Violent Crisis Intervention certification, or willingness to obtain.
- Successful completion of required background screening, including a Vulnerable Sector Check.

- Ability and willingness to undergo applicable screening and background checks successfully that are satisfactory to The Salvation Army, in its sole discretion (i.e., The Salvation Army Abuse Registry) and provide an original copy of a Background Check screening.
Compensation:
The target hiring range for this position is $24.47 to $30.59 with a maximum of $36.71.
Placement in the salary range will be based on factors such as market conditions, internal equity, and candidate experience, skills, and qualifications relevant to the role.
