Clinical Nurse Specialist (CNS) | Contract / PRN | Michigan

Guardian Home Care is seeking a Clinical Nurse Specialist (CNS) for a flexible contract / PRN opportunity supporting our Michigan home care services. This role will primarily involve completing clinical assessments, clinical packets, and related documentation as needed.

This opportunity may be a good fit for an experienced CNS seeking supplemental work or considering a transition to a new agency.

Responsibilities

Complete clinical assessments and evaluate clients care needs.
Prepare accurate, thorough clinical packets and supporting documentation.
Complete applicable medical needs certification forms within professional scope and program requirements.
Review clinical information and identify additional documentation needed to complete assessments.
Communicate with the agencys clinical and administrative teams to support timely completion of required paperwork.
Maintain client confidentiality and follow applicable documentation standards.

Required Qualifications

Active Michigan Registered Nurse (RN) license.
Masters degree or higher in nursing.
Clinical Nurse Specialist (CNS) certification.
Active National Provider Identifier (NPI) number.
Strong clinical assessment, documentation, and organizational skills.
Ability to work independently and communicate effectively with the agency team.

Preferred Qualifications

CHAMPS Provider ID number. Assistance with obtaining one is available for candidates who are not currently enrolled.
Experience completing clinical assessments, medical needs certifications, or similar documentation.
Experience serving clients in home care, community-based care, or Medicaid-funded programs.
Familiarity with Michigans Home Help program.
