We are currently hiring a Specialty Program- RN, based in Worcester, MA. This role will work as part of a collaborative healthcare team to provide high-quality primary care, with a focus on gender-affirming care. The RN assesses, treats, and educates patients within the scope of nursing practice, while coordinating care and providing support to patients and their families. This role works closely with other members of the clinical practice team to ensure patients receive comprehensive, coordinated care.

Essential Functions
- Provides direct patient care within the scope of licensure, including assessments, vaccinations, medications, and treatments.
- Provides injection training and medication education to patients.
- Supports clinical documentation for medication prior authorizations and pre-surgical coordination as directed by providers.
- Delegates appropriate patient care activities to support efficient patient flow.
- Coordinates care with external providers and services, including medication management, diagnostic results, ER visits, and specialty consultations.
- Performs point-of-care (POC) testing as needed.
- Documents patient care in the EHR according to health center and nursing standards.
- Adapts patient care based on age and individual patient needs.
- Follows all organizational policies, procedures, and standards related to nursing practice.
- Triages patients with acute symptoms, both in person and by telephone.
- Requests medical interpretation services as needed.
- Assists with training new nursing staff and may precept nursing students.
- Inspects standard stock medications, emergency medication supplies, and oxygen equipment weekly.
- Supports management of the vaccine program as needed.
- Performs additional duties as assigned, including quality assurance activities and patient follow-up and tracking.
Education and Experience
- Current Massachusetts Registered Nurse (RN) License
- Associate’s or Bachelor of Science in Nursing (BSN) degree from an accredited college or university
- Current Basic Life Support (BLS) certification
