About the Role:

Snapscale is looking for a reliable, detail-oriented Healthcare Virtual Assistant (HVA) to serve as a Case Manager supporting a US-based primary care and urgent care practice network. In this role, you will be responsible for patient outreach, scheduling, coordinating care between providers and labs, and obtaining necessary clinical records to support patient care and quality metrics.

Responsibilities:

- Call patients to schedule appointments and follow-up visits
- Perform patient outreach to confirm whether required health screening services have been completed
- Identify healthcare facilities or providers where completed screening services were performed
- Request, follow up on, and obtain medical records for completed screening services
- Coordinate care between providers, labs, and patients
- Assist with patient intake, registration, and managing patient inquiries and FAQs
- Update and maintain patient records and clinical documentation within the EMR system
- Support quality programs and health outcome workflows (such as ACO and HEDIS measures)
- Provide additional healthcare administrative support as assigned

Requirements

- Previous experience as a Healthcare Virtual Assistant, Medical Virtual Assistant, Case Manager, Clinical Medical Assistant, Referral Coordinator, or similar healthcare administrative role
- Proven ability in patient outreach, care coordination, and obtaining medical records
- Exceptional attention to detail and strong administrative follow-through
- Strong written and verbal English communication skills
- Experience working with EMR/EHR systems (such as AdvancedMD), VoIP phone systems, and Microsoft Office / Office 365
- Able to work independently while maintaining consistent productivity and accountability
- Must be willing to work onsite in Davao
