Position Overview

MindCare Solutions is seeking a qualified Nurse Practitioner (APP) to provide comprehensive medical and behavioral health services within a Kentucky correctional facility. The APP will provide evaluation, diagnosis, treatment, and ongoing management of patients while working collaboratively with nursing staff, physicians, and the multidisciplinary care team.

Responsibilities

- Conduct comprehensive patient assessments, including medical and psychiatric evaluations as appropriate.
- Diagnose and develop individualized treatment plans.
- Provide ongoing management of acute and chronic medical and behavioral health conditions.
- Prescribe and manage medications within the scope of licensure and facility protocols.
- Respond to urgent and emergent patient needs.
- Monitor patient progress and adjust treatment plans as clinically appropriate.
- Collaborate with physicians, nursing staff, behavioral health providers, and other members of the healthcare team.
- Provide patient education regarding diagnoses, medications, treatment plans, and preventative care.
- Maintain timely and accurate clinical documentation.
- Participate in multidisciplinary case reviews and care coordination.
- Follow all applicable correctional facility policies, clinical standards, and regulatory requirements.
- Provide care in a professional, trauma-informed, and patient-centered manner within the correctional environment.

Qualifications

- Current, unrestricted Kentucky Nurse Practitioner/APRN license .
- National board certification as a Nurse Practitioner in the applicable specialty.
- Active DEA registration , if required for the position.
- Experience in correctional, behavioral health, primary care, emergency, or other high-acuity settings preferred.
- Strong clinical assessment, communication, and documentation skills.
- Ability to work effectively within a multidisciplinary healthcare team.
- Must successfully complete required background screening, credentialing, and facility clearance requirements.
