As a Registered Nurse, you’ll contribute to our success in the following ways:

- Assess patients’ physical, psychological, social, and spiritual needs and incorporate findings into the Plan of Care.
- Complete initial evaluations and reassess nursing needs during follow-up visits based on patient, family, and facility needs.
- Monitor patient condition and promptly communicate significant changes, concerns, or safety issues to the physician and care team.
- Update the Plan of Care as patient needs change and coordinate revisions with the appropriate care team members.
- Educate and support patients, families, caregivers, and facility staff on the Plan of Care, hospice philosophy, and pain and symptom management.
- Collaborate with the Medical Director, attending physician, branch leadership, facility staff, and interdisciplinary care team to coordinate patient care.

What is Required?

- Current State license as a Registered Nurse.
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
- 1+ year of experience as a nurse in a clinical care setting.
- Ability to handle multiple priorities, documentation requirements, and deadlines
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
- Experience in hospice or home health environment is preferred
