Job Summary and Responsibilities
As our Patient Placement RN, you will be responsible for ensuring a safe and efficient admission or transfer process into our hospital. Your primary goal is to optimize patient flow and access to care.Every day, you will work collaboratively with a diverse group of stakeholders, including physicians, patient transportation providers, and nursing staff at referring hospitals and practices. You will evaluate appropriate patient placement and facilitate rapid transfers, ensuring seamless transitions for patients requiring our services.To be successful in this role, you will possess exceptional communication and critical thinking skills, a deep understanding of patient care needs, and the ability to coordinate complex logistics under pressure. Your commitment to patient safety and efficient healthcare access will be essential in achieving our mission.
- Evaluates, process, tracks, documents and coordinates patient transfers and direct admissions into the CHI Saint Joseph Healthcare system.
- Obtains appropriate medical and demographic information from sending provider.
- Coordinates communication between referring physicians, and the accepting physician.
- Ensures compliance with current EMTAL and Cobra guidelines.
- Possesses strong communication skills and conflict resolution skills.
- Charge RN delegation as assigned by Access Center Manager with duties to include but not limited to the focus on Quality, Service Excellence, Finance Growth and Engagement. See Charge RN duties and responsibility attachment located on the Access Center shared drive.
Job Requirements
Required
- Bachelors Of Nursing Nursing, upon hire or
- Bachelors Of Science currently in school obtaining BSN and 4-6 years with greater than five years acute care experience., upon hire or
- Associate Of Nursing currently in school obtaining ADN and 4-6 years with greater than five years acute care experience., upon hire and
- 1-3 years Minimum two years of experience as an RN in the acute care setting. , upon hire
