How you’ll make an impact in this role
- Lead the development, coordination, and ongoing refinement of individualized care plans for high-risk, high-need patients across multiple payer populations.
- Support patients in identifying and defining personal health goals that enhance self-management, stability, and confidence in their overall healthcare journey.
- Collaborate closely with interdisciplinary partnersâincluding RN Care Managers, providers, social services, and community agenciesâto ensure alignment of care plans and timely communication on patient status.
- Serve as a strong patient advocate by identifying resource gaps, addressing behavioral health and SDoH needs, and escalating concerns to ensure continuity and quality of care.
- Use clinical expertise to reduce avoidable admissions and readmissions, proactively intervening with patients at highest risk across all Ascension markets.
- Within the scope of this role, the Behavioral Health Care Manager must demonstrate strong critical-thinking skills, sound clinical judgment, and the ability to adapt to the changing and diverse needs of the populations served. The position requires the ability to work effectively in a fast-paced environment, along with experience documenting in an electronic health record and proficiency with the basic computer skills necessary for remote work.
What minimum requirements you’ll need
Licensure / Certification / Registration
- Required Credential(s): None Required.
- Preferred Credential(s): BLS Provider. American Heart Association or American Red Cross accepted.
- Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC).
- Counselor.
- Social Worker.
Education
- High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
Preferred Experience:
- Experience: 3â5 years of experience in care management, case management, social services, community health, or clinical practice, required.
- Core Knowledge and Technical Proficiency: Knowledge of care coordination fundamentals, interdisciplinary collaboration, behavioral health, social determinants of health, and community resources, along with proficiency in electronic health records and care management software, required.
- Certification & Specialized Experience: Certification in Case Management (CCM, ACM, or similar), and experience within managed care, population health, or value-based care environments
