POSITION SUMMARY:
The Licensed Practical Nurse (LPN) in the Discharge Lounge plays a key role in supporting safe, efficient, and patient-centered transitions from the hospital to home or another level of care. The LPN is responsible for coordinating and facilitating patient flow within the Discharge Lounge, monitoring patients awaiting transportation, medications, and promptly identifying and responding to changes in patient condition.
Working collaboratively with inpatient units, procedural areas, transportation services, pharmacy, and interdisciplinary team members, the LPN helps ensure a seamless discharge experience while maintaining patient safety, comfort, and satisfaction.

ESSENTIAL RESPONSIBILITIES / DUTIES:
Patient Care and Monitoring
- Receive and support patients transferred to the Discharge Lounge from inpatient units, procedural areas, and ambulatory care settings.
- Monitor patients while they await transportation, medications, family pickup, or transfer to another facility.
- Perform ongoing observation of patients within the scope of LPN practice.
- Recognize, respond to, and escalate changes in patient condition to the appropriate provider or sending unit nurse.
- Initiate emergency response procedures and obtain assistance as appropriate.
- Facilitate the safe and efficient flow of patients through the Discharge Lounge.
- Verify completion of discharge requirements prior to patient arrival, including discharge orders, After Visit Summary (AVS), patient education, and medication reconciliation completed by the sending unit.
- Collaborate with pharmacy staff regarding medication delivery and communicate any concerns to the sending unit.
- Coordinate patient transportation, including family pickup, ride services, wheelchair assistance, ambulance transportation, or other discharge arrangements.
- Maintain a clean, organized, safe, and welcoming environment for patients and families.
Care Coordination and Communication
- Communicate effectively with inpatient nursing units, procedural areas, case management, pharmacy, transportation services, and other members of the healthcare team.
- Provide updates regarding patient status, transportation delays, or discharge barriers.
- Escalate concerns that may impact patient safety, discharge efficiency, or patient experience.
Working Conditions
- Fast-paced clinical environment requires frequent communication and coordination across multiple departments.
- Requires prolonged standing, walking, pushing wheelchairs, and assisting patients with mobility as needed.
- May care for patients with varying medical, surgical, and psychosocial needs while awaiting discharge.

Job Requirements
EDUCATION:
Graduate of an accredited (LPN) Practical Nursing program.
CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED :
- Current Licensed Practical Nurse (LPN) license in good standing in the Commonwealth of Massachusetts.
- Basic Life Support (BLS) Healthcare Provider certification
KNOWLEDGE AND SKILLS:
- Promote patient safety and satisfaction throughout the discharge process.
- Support organizational goals related to patient flow, throughput, and length of stay reduction.
- Demonstrate professionalism, accountability, and customer service excellence.
- Maintain compliance with hospital policies, regulatory requirements, and nursing standards of practice.
- Proficiency in EPIC and standard Microsoft Office applications, including Word, Excel, Outlook, and PowerPoint.
