Position: Infusion Field Nurse (RN) 
Reports to: Clinical Supervisor 
Compensation: $100-$160 per visit (~2 hour min)
Schedule: Part-Time 
Location: Bay Area/North Bay
 
About the Role
We are seeking a Registered Nurse (RN) specializing in infusion therapy and wound care to provide continuous, one-to-one nursing care in the patient’s home. This role is ideal for an RN who operates with great attention to detail and a big heart, delivering high-quality, compassionate care while remaining attentive to changes in the patient’s condition.
 
In this role, you will focus on direct patient care, comprehensive assessment, and clinical escalation within the patient’s home environment. You serve as the primary link between the patient and the clinical team, ensuring continuity of care and clinical excellence in accordance with:
• California Title 22 HHA regulations and CDPH standards.
• Accreditation Commission for Health Care (ACHC) standards.
• CMS, VA/TRICARE, Medi-Cal/CCS, and private insurance requirements.

Key Responsibilities
1. Clinical Excellence & Direct Patient Care 
• Perform initial and ongoing comprehensive assessments and health histories to determine individual home care needs.
• Deliver skilled nursing care, including IV therapy, IVIG (intravenous immunoglobulin) therapy, IV medication and fluid administration, vascular access, and line maintenance, while following prescribed treatment plans and strict aseptic techniques.
• Provide and manage care for peripheral IVs, PICC lines, midlines, and central lines, as applicable to patient needs.
• Monitor patients before, during, and after infusions, including IVIG therapy, for infusion-related reactions, complications, adverse effects, and changes in clinical condition, taking appropriate action and notifying the care team as needed.
• Perform comprehensive wound assessments, including wound measurement, evaluation of wound characteristics, drainage, surrounding tissue, and signs of infection or delayed healing, as applicable to patient needs.
• Provide ordered wound treatments and dressing changes, document wound status and patient response, and manage wound VAC/negative-pressure wound therapy when applicable. 
• Evaluate the home environment to ensure it is safe and appropriate for infusion therapies, equipment, and wound care management.
• Initiate and revise the plan of care based on changes in patient status and needs.
• Educate patients and families on their treatment plans, infection prevention, and signs or symptoms requiring medical attention, providing guidance to support safe and stable care in the home.
• Function as a case manager/supervisor when assigned by the Clinical Supervisor.

2. Communication & Care Coordination
• Prepare timely and accurate clinical documentation and update the primary physician regarding changes in patient condition in accordance with regulatory and organizational requirements.
• Communicate directly with physicians and other healthcare personnel regarding patient condition, treatment response, infusion-related concerns, wound status, orders, and medication or dressing changes.
• Report significant changes in patient condition, signs of infection, delayed wound healing, infusion complications, or other clinical concerns to the appropriate members of the care team.
• Identify discharge planning needs during care plan development and implementation.
• Supervise and evaluate home health aide care provided every two (2) weeks when assigned.
• Collaborate with the interdisciplinary care team to support continuity and coordination of patient care.

3. Compliance & Administrative Standards
• Ensure all necessary equipment, supplies, medications, and materials required for infusion therapy, vascular access, and wound care are available and appropriate for patient care.
• Maintain accurate and timely clinical documentation in accordance with organizational, regulatory, and payer requirements.
• Participate in on-call duties as defined by agency policy.
• Maintain strict compliance with CMS, ACHC, and Title 22 standards, as well as applicable infection prevention, medication safety, and clinical practice requirements.

Position Qualifications:
• Active and unrestricted California RN license.
• Current CPR/BLS certification.
• Graduate of an accredited school of nursing; BSN is preferred (not required).
• Minimum of one year of professional nursing experience.
• Experience working in home health, acute care, infusion, or related clinical experience is preferred.
• Demonstrated hands-on experience in IV therapy and vascular access required.
• Experience in administering IVIG therapy is highly preferred.
• Experience with peripheral IVs, PICC lines, midlines, and central lines, as applicable.
• Wound care experience, including wound assessment, dressing changes, wound measurement, documentation, and infection monitoring.
• Experience with wound VAC/negative-pressure wound therapy preferred.
• Wound care certification (WCC, CWS, or CWCN) and infusion certification (CRNI) are preferred but not required.
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; with competency appropriate to patient acuity.
• Prolonged or considerable walking or standing. Able to lift, position or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing to perform required nursing skills.
• Licensed driver with a reliable automobile in good working condition and insurance meeting organizational requirements.
