Are you passionate about shaping the future of post-acute and home-based healthcare while delivering exceptional experiences for members, providers, and business partners? CenterWell OneHome is seeking an accomplished Product Manager (OneHome Product Development) to join our Product Strategy and Development team.
In this role, you will drive innovation by leading product research, discovery, and strategy efforts in partnership with internal stakeholders and external collaborators. Working closely with Product Leadership, you will identify opportunities to improve business processes, enhance operational performance, and develop scalable solutions that advance OneHome's mission.
You will play a key role in the conception, development, and evolution of products and capabilities that address the changing needs of health plans, providers, members, and caregivers. Success in this role requires strategic thinking, strong business acumen, effective stakeholder management, and a passion for improving healthcare outcomes.

What You Will Own

As a Product Manager, you will embody and champion the principles that drive OneHome's product culture and long-term success:

- Customer Obsession : Develop a deep understanding of the needs, challenges, and experiences of members, providers, caregivers, and health plan partners. Use customer insights and data-driven decision-making to identify opportunities and deliver meaningful solutions that create measurable value.
- Long-Term Strategic Thinking : Balance immediate business priorities with long-term organizational goals by identifying emerging market trends, healthcare industry changes, and growth opportunities that strengthen OneHome's competitive position.
- Innovation and Continuous Improvement : Demonstrate curiosity and a willingness to invent by challenging existing processes, exploring new ideas, and developing innovative products, services, and capabilities that enhance care delivery and business performance.
- Operational Excellence : Drive disciplined execution through effective planning, prioritization, performance measurement, and cross-functional collaboration. Ensure solutions are scalable, sustainable, and aligned with operational, financial, and technology objectives.
- Product Ownership and Accountability : Lead products through their lifecycle, from discovery and strategy through implementation and optimization, while maintaining accountability for business outcomes, stakeholder alignment, and customer satisfaction.
- Stakeholder Leadership and Influence : Build trusted relationships across business, operations, technology, and external partner teams to align priorities, facilitate decision-making, and achieve shared objectives.
- Data-Driven Decision Making : Leverage qualitative and quantitative insights to evaluate opportunities, measure success, assess trade-offs, and prioritize investments that deliver the greatest impact.

​ Key Responsibilities

- Lead product discovery, strategy, and business analysis efforts to identify opportunities for growth, efficiency, and innovation.
- Evaluate and optimize end-to-end business processes, translating strategic objectives into scalable products, services, and capabilities.
- Partner with business, operations, technology, and external stakeholders to align priorities and deliver business value.
- Develop business cases, cost-benefit analyses, and strategic assessments, including build, buy, and partner evaluations.
- Manage products throughout their lifecycle, from concept and planning through implementation and continuous improvement.
- Facilitate executive and cross-functional discussions to drive alignment, decision-making, and action planning.
- Support the expansion and enhancement of OneHome services, including skilled nursing facilities, home health, durable medical equipment (DME), and other post-acute care offerings.
- Drive initiatives that improve member outcomes, provider experience, care coordination, and operational performance.

Required Qualifications

- Bachelor's degree in Business, Healthcare Administration, Public Health, Information Systems, or a related field.
- 5+ years of experience in product management, healthcare operations, consulting, project management, post-acute care, home health, payer, provider, or healthcare technology environments.
- Proven success managing products from concept through delivery, including strategy development and execution.
- Experience conducting complex business analysis and translating business needs into scalable solutions.
- Strong understanding of healthcare delivery models, provider operations, care coordination, and healthcare business processes.
- Exceptional communication, analytical, stakeholder management, and relationship-building skills.
- Demonstrated ability to manage multiple priorities and deliver results in a collaborative environment.

Preferred Qualifications

- Master's degree in Business Administration, Healthcare Administration, Public Health, Health Informatics, or a related field.
- Professional certifications such as CSPO, Certified Product Manager, SAFe Product Manager/Product Owner, PMP, CBAP, or Lean Six Sigma.
- Experience with post-acute care, home health, value-based care, care management, utilization management, or provider network products.
- Experience working with Medicare Advantage, Medicaid, dual-eligible populations, or home-based care delivery models.
- Experience collaborating with third-party vendors, healthcare partners, and strategic service providers.
- Proven ability to lead through ambiguity and navigate complex healthcare environments.

Work at Home Requirements: To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested. In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs. Humana reserves the right to require associates to upgrade their internet service if necessary. Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.
Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$104,000 - $143,000 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
