Medical Assistant (MA) IBH Care Coordinator
The MA IBH Care Coordinator provides psychoeducation and case management follow-up and referral services to improve patient health outcomes. The MA IBH Care Coordinator directly supports Behavioral Health providers, including Behavioral Health clinicians (Psychologists, LCSWs, LMFTs) and Psychiatric Nurse Practitioners, by performing vital signs assessment and conducting case management related to medication monitoring, behavioral activation, maintenance, and connecting patients with community resources. The MA IBH Care Coordinator contributes equally to patient care teams through huddles, case consultations, patient advocacy, mental health education, and collaborative meetings with medical staff. The MA IBH Care Coordinator is expected to be professional, punctual, maintain regular attendance, and demonstrate cooperation, organization, and enthusiasm at all times.

Required Qualifications
- High School Diploma or equivalent.
- Must have a Medical Assistant training certificate of completion.
- Current CPR or CPR/AED certification required.
- Experience and/or education in human services-related fields such as social work, psychology, counseling, alcohol and drug treatment, or chemical dependency.
- Bilingual in Spanish preferred; other languages are a plus.
- Demonstrated commitment to serving underserved populations and working effectively with individuals from diverse educational, socioeconomic, ethnic, cultural, and lifestyle backgrounds, including those experiencing homelessness, substance use disorders, HIV/AIDS, disabilities, or other health challenges.
- Strong verbal and written communication skills and active listening abilities.
- Ability to establish and maintain positive, professional relationships.
- Exceptional interpersonal skills.
- Excellent customer service skills, including active listening, problem-solving, and maintaining composure during stressful or emotional situations.
- Commitment to HIPAA compliance and maintaining patient confidentiality.

- Compliance with applicable state and federal laws and regulations.
- Proficiency with Microsoft Office applications, including Word, Excel, PowerPoint, Outlook, and database software.
- Adherence to infection control and workplace safety procedures.
- Assist and support the department in meeting High Reliability standards.
- Must be willing to successfully complete the credentialing process.
Essential Responsibilities
- Conduct warm hand-offs between patients and referring primary care providers during clinic hours to connect patients with Behavioral Health services.
- Assist patients in navigating the mental health system and accessing appropriate county agencies, community programs, and support resources.
- Monitor and implement Patient-Centered Health Home (PCHH) goals.
- Monitor and implement Certified Community Behavioral Health Clinic (CCBHC) goals.
- Perform Medical Assistant duties, including preparing exam rooms, rooming patients, recording medical histories, taking vital signs, administering immunizations, and providing medication-related assistance within scope of practice.
- Document all patient interactions in the Electronic Health Record (EHR) according to organizational policies and documentation standards.
- Assist with data collection, reporting, projects, quality initiatives, utilization reviews, and other related activities.
- Travel as needed to support operational requirements, meetings, and organizational functions.
- Maintain immunization records for county registry requirements.
- Maintain confidentiality and protect privileged information related to patients, employees, and the organization.
- Perform duties within the Medical Assistant scope of practice.
- Perform other duties as assigned.
Job Type & Location This is a Contract to Hire position based out of Citrus Heights, CA.
Pay and Benefits
The pay range for this position is $27.40 - $27.40/hr. Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
