About this role

Position Summary
- Provides assessment and reassessment of clients receiving hands-on personal care services, supervises In-home Aides and Client care.
- Assists with agency Quality Assurance as assigned.
- Responsible for supervision of Client care and communication with other appropriate team members. Provides supervision of In-Home Aides during all hours while care is provided and validates In-home Aide competencies by demonstration.
- Responsible for following agency policy duties that a Registered Nurse may perform.
- May also serve as the Agency Director. For In-Home Aide clients, the RN Supervisor will visit the client every 90 days. One supervisory visit annually must be in presence of the Caregiver providing care.
- Responsible for the initial admission assessment and the development of a plan of care in collaboration with the client. The plan of care is incorporated into the Client’s service record.
Education/Qualifications
- Current, valid Registered Nurse License with the NC Board of Nursing or an affiliated Nurse Licensure Compact State.
- Must have a criminal background check and maintain current CPR certification.

Reports to work as scheduled and is regular in attendance.

- Reviews schedule weekly and accepts assignments or duties as directed by the Director.

Documents according to established agency procedure.

- Documents according to agency and payer policies. Submits all paperwork into office per agency policy.

Maintains a clean and safe working environment.

- Follows all standards for universal precautions, infection control, and agency policies when reporting to assignments.

Maintains an accurate accounting of supplies.

- Follows agency procedure regarding charges and accountability for patient supplies.

Registered Nurse Assessment

- Follows all policies of the Division of Medical Assistance re: client assessment and reassessment for PCS services and documentation requirements.
- Responsible for the client initial admission assessment and reassessments. Develops the plan of care in collaboration with the client. The plan of care is incorporated into the Client’s service record.
- The RN Supervisor will visit the client every 90 days. One supervisory visit annually must be in presence of the Caregiver providing care.
- Reassesses the client and revises the client’s plan of care as needed, based upon changes in the client’s condition, situation, and environment.

Registered Nurse Supervision of In-Home Aides

- The RN Supervisor will follow DHSR licensure rules and payer source requirements for frequency of supervision. Supervision must be provided by an RN.
- Responsible for supervision of Client care and communication with other appropriate team members. Provides supervision of In-Home Aides during all hours while care is provided and validates In-home Aide competencies by demonstration.

Registered Nurse Supervision of In-Home Aides

- The RN Supervisor will follow DHSR licensure rules and payer source requirements for frequency of supervision. Supervision must be provided by a RN.
Hours: Must be able to work a flexible work schedule.

Transportation
- Must have a valid North Carolina Driver’s license and appropriate automobile liability insurance.
- Must have dependable transportation and ability to travel on the job.

Note: Must complete the Criminal Background check as per NC GS 131E 265; must complete the health screening, testing and immunization requirements of Addison Home Care Services LLC; must complete any other applicable employment screening checks such as license status.

Must have work status verified by E-Verify per agency personnel policy as applicable. Must not be listed on the North Carolina Medicaid Provider Termination and Exclusion list or the United States Department of Health and Human Services (US DHHS) Office of Inspector general (OIG) list of excluded individuals/entities upon hire or during monthly checks per agency personnel policy.
