Harvard University Health Services (HUHS), part of University Central Administration (CADM), is the on-site multi-specialty medical practice and health administration for the Harvard community. It provides care to students, faculty, staff, eligible post-docs, retirees, and their dependents. HUHS also manages two university-owned health plans, HUGHP and SHIP, and collaborates with other departments to address public health needs.
Job Description
Supports HUHS patients and their families in coordinating medical, social, emotional, insurance, and vocational services related to illness or injury. Conducts psychosocial assessments in person or by telephone and develops comprehensive outpatient care plans to support safe, effective, and timely care.
Job-Specific Responsibilities
- Collaborate with physicians, nurses, support staff, community agencies, and other healthcare professionals to coordinate patient care.
- Communicate regularly with treating clinicians regarding patient concerns, progress, and clinical updates.
- Provide patient and family support related to medical, social, emotional, insurance, and vocational needs.
- Document assessments, care plans, interventions, and updates accurately and promptly in the electronic medical record.
- Apply case management principles to promote cost-effective, high-quality, and continuous care.
- Consult with the manager regarding clinical, legal, ethical, and risk-management issues.
- Participate in quality assurance and performance-improvement activities in accordance with HUHS standards.

- Practices cultural humility and provides individualized, equitable care across differences in race, ethnicity, socioeconomic status, age, disability, language, health literacy, gender, sexual orientation, gender identity, family structure, and other aspects of identity.
- Incorporates patient-defined goals, strengths, preferences, values, and lifestyle considerations into care planning, referrals, and resource selection.
- Protects privacy, dignity, and confidentiality and tailors interventions to each patient’s needs, capacity, support system, and expressed wishes.
Qualifications
Basic Qualifications:
- Master’s degree
- Minimum of 5 years’ relevant work experience
Additional Qualifications and Skills:
- Knowledge of Microsoft Office Suite, intermediate Excel skills
- Excellent organizational, analytical, problem solving and interpersonal skills
- Experience in a hospital or healthcare setting preferred.
- Knowledge of discharge planning and care coordination preferred.
- Strong communication, collaboration, organization, and clinical assessment skills.
Certificates and Licenses:
- Licensed Independent Clinical Social Worker or LSCW with current Massachusetts license
Additional Information
Appointment End Date: This is a 6-month term position that ends 6 months from the date of hire.
Standard Hours/Schedule: 6-month term position requiring 32 hours per week, scheduled Monday through Friday between 8:00 a.m. and 5:00 p.m. Flexible scheduling may include four 8-hour days.
