About this role
Role: Coordinates communication, services, and all levels of support for assigned individuals.
Major Duties and Responsibilities:
- Participate in and provide information for annual eligibility assessment.
- Coordinate and communicate with relevant Managed Care Organizations for all care plan meetings and updates.
- Develop Person Centered Support Plans (PCSPs) while meeting KDADS timelines and expectations.
- Coordinate services derived from the PCSP, with routine follow-up reviews of progress and outcomes.
- Develop and oversee implementation of Positive Behavior Support Plans.
- Communicate with individuals served and their families to assist in oversight and coordination of services.
- Work with residential and day service program managers to address any concerns related to services and care.
- Assist with mental health and medication administration meetings to ensure accuracy and implementation.
- Attend all required meetings.
- Complete required billing documentation daily .

Qualifications
Knowledge and Skills:
- Experience: Two years prior experience in the field of human services/developmental disabilities preferred.
- Education: Bachelors degree in human services related major.
- Interpersonal Skills: Strong verbal, written and negotiation communication skills required. Ethical decision constructive, and proactive decision-making skills are required. Must consistently exhibit unfailing compassion for individuals with disabilities.
- Other Skills: Basic mathematical and intermediate computer skills required. Experience with Microsoft Office Products preferred.
- Physical/Environmental: Ability to lift a minimum of 35 pounds. Travel to appointments and conferences as needed. Moderate noise such as would be found in a business office with computers and printers, light traffic.
