As a Intake Coordinator, you will play a vital role in engaging families at critical moments and connecting them with services that promote safety, stability, and well-being. This dynamic position offers the opportunity to work directly with families during or immediately following Child Safety Conferences, helping to build trust and facilitate timely access to support. Responsibilities:
- Acts as the liaison between CMCS' and Administration for Children's Services (ACS) for all referrals and/or potential referrals
- Responds to ACS' utilization and capacity inquiries within 24 business hours
- Determines programmatic eligibility for all potential referrals
- Oversees the entry of all referrals in the PROMIS system of record
- Represents CMCS at Child Safety Conferences when the conference serves as the point of referral to Prevention Services.
- Uses motivational interviewing techniques to engage families in preventive services
- Maintains communication with ACS and key program staff throughout the engagement period
- Completes all required new intake paperwork within the required timeframes
- Opens cases at the frequency necessary to support achievement of ACS annual case opening targets for both Prevention Programs.
- Develops an understanding of the family's history and needs through consultation with the referral source and family and review of relevant systems of record including CNNX.
- Participates in the development of a comprehensive service plan that addresses service needs
Requirements:
- BSW or Bachelor's Degree in Human Services
- Minimum of 1 year working experience in an ACS contracted preventive service program
- Must have completed motivational interviewing and safety and risk training via the workforce institute
- Working knowledge of CONNECTIONS and PROMIS
- Strong interpersonal skills
- Excellent verbal and written communication skills
- Excellent time management
Fluency in Spanish and English language required
