Position Summary
As an integral member of the Care Delivery Integration (CDI) team, the RN collaborates with providers and interdisciplinary team members to coordinate patient-centered care, improve quality outcomes, close care gaps, and enhance patient experience. The RN provides clinical oversight, guidance, education, and support to Medical Assistants and clinical support staff while serving as a resource for nursing practice, standing orders, patient safety, medication management, vaccine program oversight, infection prevention, regulatory compliance, and quality improvement initiatives. The RN maintains competency in direct patient care while promoting clinical excellence, standardized workflows, staff mentoring, collaboration, education, and continuous improvement in support of organizational goals.

Qualifications
ESSENTIAL SKILLS AND EXPERIENCE:
- Bachelor’s Degree from accredited university or foreign equivalent.
- License with Arizona State Board of nursing with a license in good standing.
- Minimum two (2) years of ambulatory or acute care nursing experience.
- Demonstrated understanding of clinical methods and techniques used in record-keeping systems.
- Advanced computer efficiency in Microsoft Office suite (Word, Excel, PowerPoint, Outlook) and experience in Patient Management System
- Analyze data to identify areas of opportunity to develop appropriate action plans to meet the organization’s goals.
- Meet the organization immunization requirement
- Excellent verbal and written communication skills, including strong organizational, detail and interpersonal skills;
- Demonstrated ability to work effectively with men and women of diverse races, ethnicities, ages and sexual orientations in a multicultural environment.
- Current BLS/CPR certification.
- Arizona Level One Fingerprint Clearance Card (where required).
- Valid driver's license and ability to travel between health centers.
- Experience with Electronic Health Records.
- Knowledge of HRSA, Joint Commission, OSHA and regulatory standards.
POSITION REPONSIBILITIES
Direct Patient Care
- Perform nursing assessments, triage, medication administration, treatments, immunizations, and patient education.
- Assist providers with clinical procedures.
- Perform point-of-care testing and diagnostic procedures.
- Coordinate care for high-risk patients and patients with chronic conditions.
- Support transitional care management following emergency department visits and hospitalizations.
- Review critical laboratory and diagnostic results and provide patient follow-up.
- Develop patient-centered self-management goals.
- Ensure complete and accurate documentation within the electronic health record.
- Demonstrate expertise in department-specific clinical areas, including but not limited to: Women’s Health, such as non-stress test performance and interpretation; Pediatrics, such as immunizations, well-child checks, and breastfeeding education; and Family and Internal Medicine, such as Medicare wellness exams and other clinical services as required.
Clinical Collaboration
- Serve as the clinical subject matter expert (SME) and resource for Medical Assistants and clinical support staff.
- Provide clinical oversight and guidance regarding patient care, standing orders, nursing practice, and clinical workflows.
- Support onboarding, mentoring, and competency validation of Medical Assistants.
- Reinforce evidence-based practices, continuous quality improvement, and standardized workflows.
- Collaborate with Practice Administrators to improve clinical operations and workflow efficiency.
- Support implementation of organizational clinical initiatives.
- Identify opportunities for clinical process improvement and escalate concerns to the Nurse Manager and other key stakeholders.
Vaccine and Medication Program Oversight
- In collaboration with clinical supervisor and vaccine SMEs provide clinical oversight of vaccine storage, handling, administration, and documentation.
- Support compliance with Vaccines for Children (VFC), Vaccines for Adults (VFA), and organizational immunization requirements.
- In collaboration with the clinical team monitor vaccine storage equipment, Digital Data Loggers (DDLs), and temperature documentation.
- Ensure Medical Assistants receive proper education regarding immunization best practices.
- In collaboration with Medical Assistant’s supervisor, monitor medication safety, storage, handling, and administration practices.
- Participate in vaccine audits and regulatory readiness activities.
Quality Improvement and Regulatory Compliance
- Lead clinical quality improvement initiatives.
- Support achievement of UDS, HEDIS, Value-Based Care, and other organizational quality measures.
- Monitor clinical compliance with standing orders, infection prevention standards, medication safety, and National Patient Safety Goals.
- Participate in chart audits, tracers, clinical incident reviews, and corrective action planning.
- Support Joint Commission, OSHA, HRSA, and regulatory survey readiness.
- Assist with development and implementation of clinical policies, procedures, and standardized workflows.
Care Delivery Integration
- Collaborate with providers, Behavioral Health Consultants, Registered Dietitians, Integrated Care Coordinators, Referral Coordinators, Community Health Workers, Patient Navigators, and Medical Assistants.
- Promote interdisciplinary communication and coordinated patient care.
- Assist with closing preventive care and chronic disease care gaps.
- Support referral management and care transitions.
- Promote patient engagement and self-management.
- Identify barriers to care and connect patients with appropriate resources.
Professional Practice
- Practice within the Arizona Nurse Practice Act and organizational policies.
- Maintain required licensure and continuing education.
- Participate in departmental meetings, education, and quality initiatives.
- Promote a culture of professionalism, accountability, compassion, and respect.
- Perform additional duties as assigned.
NON-ESSENTIAL SKILLS AND EXPERIENCE:
- Experience in primary care or Federally Qualified Health Center (FQHC).
- Bilingual in English and Spanish.
- Experience with population health and quality improvement initiatives.

Work environment: While performing the duties of this job, the employee is exposed to weather conditions prevalent at the time when traveling to various clinic sites. The noise level in the work environment is usually moderate. The employee may be subject to health hazards (contagious diseases, bloodborne pathogens, etc.) when working in the clinic area.
