JOB TITLE: Occupational Therapist
SUPERVISOR: Director of Rehabilitation Services
HOURS PER WEEK: 40hrs/Non-Exempt
DEPARTMENT: Rehabilitation Services
HIRING BONUS: $1,000 after 90 days

PRIMARY FUNCTION: Responsible for evaluation and treatment of patients
DUTIES AND RESPONSIBILITIES: (*ESSENTIAL FUNCTIONS)
1. Provides evaluations including examining patients’ medical histories, and evaluation and measurements appropriate to patient diagnosis.*
2. Plans and prepares treatment plans describing a strategy, purpose and anticipated outcome.*
3. Administers exercises, splinting, application of physical agents (i.e.: ultrasound, electric stimulation), manual therapy, and traction.*
4. Evaluates effects of treatment at various stages and adjusts treatments to achieve maximum benefit often conferring with physician and other practitioners.*
5. Teaches patients to use assistive and adaptive devices. Instruct patients on at-home exercises.*
6. Records evaluations, daily treatment notes, monthly progress notes and discharge summaries timely and completely.*
7. Records treatment charges daily and completes charge tickets in an accurate and timely manner.*
8. Maintains confidentiality of patient information.*
9. Discharges patients from occupational therapy when goals or projected outcomes have been attained and provide appropriate follow up care or referrals.*
10. Assists in training and orientation of new staff.
11. Demonstrates effective communication methods either written, verbal, non-verbal or electronic.*
12. Listens to and acknowledges the concerns of patients and co-workers. Communicates effectively with patients and co-workers exchanging information and opinions
constructively. Treats all patients with respect.*
13. Makes effective decisions that reflect clear and logical thinking. Demonstrates flexibility in examining new approaches in response to changing organizational objectives.
Identifies and works proactively to solve problems.*
14. Demonstrates willingness and ability to go the “extra mile” in meeting customer needs. Represents the clinic professionally.*
15. Identifies and responds appropriately to both internal and external customer needs utilizing available resources. Keeps composure under stress. Is approachable and
compassionate.
16. Prioritizes work activities based on clinic demands to complete assignments within designated timeframes with little to no supervision.*
17. Demonstrates behavior that protects the safety of self/others and of the physical plant and equipment. Demonstrates environmental awareness by maintaining work
areas in clean and unobstructed order. Follows policy when accidents occur.*
18. Participate as an active team member in a patient-centered medical home.
19. Employees will not be discourteous or disrespectful to a customer or any member of the public while in the course and scope of company business.*
20. Work in a cooperative manner with management/supervision, coworkers, customers and vendors.*
21. Abide by company policies.*
22. Other tasks as assigned.
23. Maintain regular, in person, work attendance and punctuality, as scheduled.*
REQUIREMENTS:
⦁ Current Oregon Occupational Therapy License
⦁ Current CPR Certification
