Who We Are: CAMBA is a community of staff, volunteers, clients, donors, neighbors and partners who work together to build an inclusive New York City, where all children and adults have access to the resources and supports they need to thrive. We take a comprehensive approach by offering more than 180 integrated programs in: Education & Youth Development, Family Support, Job Training & Employment Support Services, Health, Housing, and Legal Services. We reach more than 72,000 individuals and families each year at our 100 locations, including 10,000 youth. CAMBA serves a diverse cross section of New Yorkers from new mothers in Brownsville to job seekers in the Rockaways. More than half of our clients are immigrants and refugees from around the globe. Over 85% of our families live in poverty, reflecting the challenges faced by nearly 1.7 million New Yorkers today.
PROGRAM(S) : Legacy Houses is a 65 unit affordable/supportive housing development located in the East Harlem section of New York City. Of the 65 units, 30 are studio apartments set aside for formerly homeless people with HIV/AIDS. CAMBA is the on-site service provider at Legacy Houses and offers case management and 24/7 security.
Position: Case Manager
Reports To: Program Director

What Does the Case Manager Do:
- Maintain professional relationships with clients and client confidentiality.
- Practice Universal Precautions/Standard Protocol & Procedures.
- Comply with any and all Federal, State, City and CAMBA security and privacy polices intended to protect the security and privacy of individually identifiable health information.
- Review all documentation establishing tenants' eligibility for program and make file copies.
- Assist tenants in completing all CAMBA intake applications and forms.
- Create and maintain tenant files.
- Conduct initial intake or assessment of tenants and tenants' families and/or periodic reassessments.
- Follow-up with tenants and with referral organizations regarding tenant contact and progress with referral organization.
- Work with tenants to break through barriers to goals and to assist tenants in advocating for themselves and in moving toward self-sufficiency.
- Recommend and implement strategies to persuade tenants to participate more fully in this process.
- Monitor tenants' progress toward their goals via regularly scheduled telephone contact and/or face-to-face visits, and document via progress notes.
- Provide all required information for weekly/monthly/quarterly/annual reports.
- Participate in case conferences.
- May recommend closing of cases in which tenants have: (a) achieved primary goals and have maintained stability for a period of months; or, (b) have not demonstrated a willingness to participate in the process (lost-to-service); or, (c) have become ineligible for services (e.g. moved out of area).
- May act as tenant liaison/tenant advocate with outside organizations regarding such matters as education, healthcare, housing, legal issues, etc.
- May prepare marketing materials for the program.
- May plan, coordinate and facilitate social/peer support events, including group facilitation for tenants.
- May input tenant data and tenant progress information into automated database.
- May reach out and market the program to the community in order to recruit tenants.
- May prescreen tenants over the telephone for eligibility and may schedule intake appointments.
- May escort tenants to appointments (educational, medical, social service, etc.)
- May assist tenants in completing applications for benefits and entitlements, and may process applications on tenants' behalf.
- May follow-up with tenants for a period of time after successful completion of their primary goals to assure tenant stability.
- Tasks may be modified, expanded and/or assigned over a period of time.
Education, Licenses and/or Certifications Required:
- Bachelor's degree (e. g., B.A., B.S.W.) and two years of applicable experience.
OTHER REQUIREMENTS:
- Flexible hours may be needed to provide shift coverage in the evenings/weekends based upon the needs of the residents.
Compensation : $50,192 annually
Status: Full-time
