What you’ll bring as Clinical Team Manager ?
- Lead clinical supervisors, office staff, and interdisciplinary teams to deliver safe, high-quality hospice care.
- Manage staffing, hiring, training, assignments, and retention to meet patient needs across branch locations.
- Set team expectations, coach staff, and address performance and operational issues.
- Oversee referrals, care coordination, documentation, and timely patient assessments.
- Ensure compliance with hospice regulations, payer requirements, privacy standards, and organizational policies.
- Partner with leadership on strategic planning, service improvements, and quality initiatives.
- Build relationships with providers, community partners, patients, and families.
What is required?
- Associate degree in a Nursing related field, Bachelor preferred
- 2 years of home care experience in Hospice environment
- 1 year of supervisory experience required
- Current, unrestricted RN license valid for the state of work
- Ability to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and have knowledge in Microsoft Office
- Able and willing to travel up to 50% as needed to support business and patient needs.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Valid driver’s license and insurance and reliable transportation to perform job tasks
You will report to the Administration or Branch Director.
