Shift:
Third Shift (United States of America)
Must have the knowledge and ability to provide comprehensive clinical pharmacy services across internal medicine, surgery, neonatal, cardiology, oncology, and neurology patient populations. This would include managing TPN therapy, anticoagulation therapies, supporting antimicrobial stewardship, performing pharmacokinetic monitoring using Bayesian and linear kinetic modeling, and optimizing medication regimens through renal dose adjustments, IV-to-oral conversions, and enteral-tube administration modifications. Must have the ability and knowledge to verify medication orders, provide evidence-based recommendations, conduct admission & discharge reconciliations, and collaborate with pharmacy residents and students.
Pharmacist Specialist. Nebraska Medicine. 600 South 42nd Street, Omaha, NE 68198. Prepare, dispense and distribute medication and provide clinical monitoring and drug information in a manner which ensures optimal patient care. Provide the highest quality patient care. Supervise pharmacy services in assigned patient care area and ensure all medication related patient and care provider needs are met. Work proactively as a member of a team(s) to ensure the provision of optimal pharmaceutical care. Demonstrate a dedication to continual and constant quality improvement.
Required Qualifications:
- Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited Doctor of Pharmacy degree program OR possess a Bachelor or Master of pharmacy degree with 10 years of experience required.
- Licensed (or eligible for licensure) in the State of Nebraska or jurisdiction in which pharmacy practice is conducted required.
- Basic Life Support (BLS) certification required, must be from an American Heart Association (AHA) endorsed program, as deemed appropriate for patient facing roles and relevant to practice.
Preferred Qualifications:
- American Society of Health-System Pharmacists (ASHP) accredited postgraduate year one residency preferred.
- Board of Pharmaceutical Specialties (BPS) certification as deemed appropriate and relevant to practice preferred.
