This is a remote/work from home position with up to 10% travel/year.

Summary
This role will manage the Operation Integration Specialists (OIS) in the administrative and clinical practice transformation programs and alternative payment models (APM) leading to an advanced level patient-centric approach that allows the organization to thrive in a value-based health care model.  Key responsibilities include proactively maintaining a thorough understanding of the requirements and regulations of the alternative payment practice transformation programs (i.e., CPC+, Ohio CPC, PCF etc.); to provide guidance and support to the team members who provide practice level guidance and assist to drive continuous quality improvement efforts through process redesign and performance management. This role ensures accountability for accurate performance of processes and workflows and is responsible for supporting the development and maintenance of high performing teams.

Essential Job Functions

- Effectively educates the OIS team on the elements, measures, tasks, and tools to be used to support various practice transformation functions and activities.  Is a professional role model and provides leadership in motivating and inspiring the team to keep projects moving toward successful completion.  Maintains thorough working knowledge of all models of care, data elements and tools/resources to support a high functioning team.
- Acts as a liaison between the OIS team and population health team, i.e., Ambulatory Care Managers (ACM) Care Transformation Coordinators (CTN), Clinical Outcome Managers (COM); including monitoring and appropriately elevating any potential issues, challenges, barriers and success stories with leadership. Appropriately communicates the organization’s vision, values, key drivers and operational needs to the team.
- Manages activities of the OIS team members including, but not limited to rounding schedules consistent and accurate communications, issue resolution, workflows for onboarding new providers and complying with evolving government reporting requirements.
- Develops and maintains the team plan to reflect the necessary resources, tasks and overall timeline needed to complete the program requirements and meet required government deadlines.
- Ensures the OIS team supports, encourages and works closely with providers and their care teams to implement process & quality improvement activities and workflows necessary to promote high quality and cost-efficient care.  Monitors and elevates any potential issues, challenges or barriers and communicates escalation as appropriate.  Supports the development of relationships with the provider network to support overall goals of the programs.
- Educates and communicates with providers as the primary resource for alternative payment practice transformation programs.
- Manages the collection, analysis, translation and reporting of quality and performance metrics. Works closely with the data analytics team and EPIC EHR team to obtain accurate and actionable information/data for use by the region to improve patient care and services, improve population health management, and reduce the total cost of care.
- Directs activities to collect, track and analyze practice demographics, transformation activities and completes regular data reporting requirements accurately for participation in federal, state, local and internal practice transformation programs by established deadlines.
- Attends required training, education and learning events by webinar, teleconference, virtual or in person.  Acts as the subject matter expert (SME) on government alternative payment models.
- Performs other duties as assigned.

Required Education:

- Bachelors in Healthcare Administration or nursing related field

Preferred Education:

- Masters in Healthcare Administration or nursing related field

Minimum experience:

- 5 years of experience in an ambulatory healthcare setting, preferably in a leadership role in a highly matrixed organization.

Other Knowledge, Skills, Abilities Required:

Subject Matter Expert in physician practice operations and practice transformation activities; Strong analytical skills; Ability to manage multiple simultaneous projects; Work independently and proactively in stressful medical environment; Attentive to details and accuracy; Able to manage time, priorities and resources; Strong verbal, written and interpersonal skills with the ability to adopt communication style to engage target audience; Experience and working knowledge of Microsoft office Word, Excel, and PowerPoint; Ability to travel locally and regionally; Ability to plan, hold and facilitate meetings and networking events.

Working knowledge of EHR’s, preferably EPIC; Working knowledge and familiarization with process improvement models (such as PDSA, A3, CQI, Lean, Six Sigma)
