Job Summary and Responsibilities
As our Social Worker, you will be a compassionate advocate, providing vital support and guidance to individuals and families facing complex challenges.Every day, you will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources. You'll advocate for clients and collaborate with multidisciplinary teams to ensure holistic support and promote well-being.To be successful in this role, you will possess exceptional interpersonal skills, empathy, strong knowledge of community resources, and proven crisis management abilities, fostering positive change and client empowerment.
- Assess, develops and implements care plans by developing appropriate response plans for participants
- Assess and make clinical evaluations and diagnose mental illness for participants
- Provide support and communication; support to social service department and make judgements on best course of treatment for participants
- Provide mental and behavioral therapy by using strength based social work approach to treat participants
- Provide counseling using applied psychotherapy of a non-medical nature with individuals or groups
- Facilitate direct therapy, make referrals or provide peer support groups
Job Requirements
Required
- MSW in Counseling Psychology
- Two (2) years of experience working with the elderly or disabled adults
- Experience in how to conduct an interview, assess, develop and implement care plans; work with groups and community agencies
- Licensed Clinical Social Worker: CA
Preferred
- MFT or MA in Counseling, Psychology
- Course work in gerontology
