General Description:
The Medication Reconciliation Pharmacy Technician is an advanced‑level role responsible for obtaining, verifying, and documenting accurate and comprehensive medication histories for patients across inpatient care settings. Working under the supervision of a licensed pharmacist, this position plays a critical role in supporting safe transitions of care by identifying discrepancies, preventing medication errors, and ensuring accurate medication information is available to the healthcare team. This role requires significant experience, strong clinical judgment, and direct patient engagement in fast‑paced clinical environments.

- Conducts thorough patient and caregiver interviews to obtain complete and accurate home medication histories, including prescription medications, over‑the‑counter products, herbal supplements, and as‑needed therapies.
- Independently verifies medication histories through contact with community pharmacies, prescriber offices, long‑term care facilities, and other healthcare entities as needed.
- Accurately documents verified medication histories, allergy information, and relevant patient data within the electronic health record in a timely manner.
- Identifies discrepancies between documented home medications and admission, transfer, or discharge orders and communicates findings promptly to pharmacists and providers for resolution.
- Collaborates closely with pharmacists, nurses, physicians, and other healthcare team members to support medication reconciliation processes across transitions of care.
- Obtains and documents additional patient information necessary to support safe medication ordering, including height, weight, and relevant clinical details.
- Applies advanced prioritization and time‑management skills to manage workload efficiently in high‑volume clinical environments such as emergency departments and admission units.
- Maintains strict patient confidentiality and complies with HIPAA and all organizational privacy requirements.
- Participates in continuous quality improvement initiatives aimed at improving medication reconciliation accuracy, workflow efficiency, and patient safety outcomes.
- Assists with training, onboarding, and support of new staff, students, or trainees involved in medication history collection processes.
- Supports pharmacy and hospital initiatives focused on reducing medication errors, improving documentation accuracy, and enhancing patient safety.
- Serves as a subject matter resource for medication history workflows and best practices, escalating complex clinical concerns appropriately.

Minimum Qualifications:
Education: Associate’s degree required.
Experience: At least five (5) years of experience as a pharmacy technician required.
License(s)/Certification(s)/Registration(s) Required: Active permit as a Pharmacy Technician by the Oklahoma State Board of Pharmacy within 6 weeks of hire. National Pharmacy Technician Certification (CPhT) required.
Knowledge, Skills and Abilities:
- Advanced knowledge of medication names, dosage forms, routes of administration, and common therapeutic uses.
- Strong understanding of inpatient medication reconciliation processes and transitions‑of‑care workflows.
- Exceptional attention to detail and commitment to data accuracy in a high‑risk clinical environment.
- Strong verbal and written communication skills, including the ability to conduct sensitive patient interviews and communicate findings clearly to clinicians.
- Ability to exercise sound judgment and independently resolve routine issues while escalating complex or clinical concerns appropriately.
- Advanced time‑management and organizational skills to prioritize competing demands effectively.
- Demonstrated professionalism and ability to build rapport with patients, caregivers, and healthcare staff.
- Proficiency with electronic health records and pharmacy documentation systems.
- Working knowledge of HIPAA and patient privacy requirements.
