Essential Functions

- Oversee up to four Physician Assistants or Nurse Practitioners, including signing off on patient charts.

- Diagnose and treat patients in private homes or assisted living facilities, including House Call services for common chronic diseases via “in-person” House Call and/or telemedicine visits

- Analyzing and interpreting patients' histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnoses

- Developing treatment plans based on standards of care and professional practice guidelines

- Chronic Medication management and educating patients on test results and treatment plans

- Ordering or performing diagnostic tests.

Requirements

- Doctor of Medicine Degree (MD) or Doctor of Osteopathy Medicine Degree (DO)

- Licensed as a physician in good standing in Arizona , additional states preferred

- Board-certified in Family Medicine or Emergency Medicine

- 2+ years of post-residency experience in primary care, family practice, urgent care or specialties

- Outstanding clinical expertise, evidenced-based practice

- Culturally responsive and competent, providing patient education at every visit

- Strong verbal and written communication skills

- Above average comfort with technology and assisting patients with troubleshooting technical difficulties that may occur in the home environment
