Job Summary
The Community Resource Specialist provides hands-on, face-to-face assistance to adults served at MHRC's South Comprehensive Services Center (CSC). The role helps individuals identify and access entitlement programs and community resources, such as Medicaid, food assistance (SNAP), Social Security benefits, and housing. The Community Resource Specialist serves individuals who walk in to the CSC and individuals referred by the System of Care (SOC) Navigator. All services follow Recovery-Oriented System of Care (ROSC) principles and use person-centered, strengths-based, and trauma-informed approaches.
Responsibilities
• Assesses the entitlement and community resource needs of individuals who walk in to the CSC or are referred by the SOC Navigator.
• Provides direct assistance with resource identification, entitlement applications, and connection to available services, using a person-centered, recovery-oriented approach.
• Assists individuals with navigating the State ACCESS system and gathering required documents to complete entitlement applications.
• Refers eligible individuals to SOAR processors for Social Security application assistance.
• Tracks pending entitlement applications, conducts follow-up, and reports outcomes to the SOC Navigator for individuals referred by SOC.
• Refers walk-in individuals who need behavioral health services, other program referrals, or follow-up after leaving the CSC to the SOC Navigator. Shares resources provided, actions taken, and outstanding needs so the SOC Navigator can close the loop.
• Maintains and updates the Community Resources shared folder, including housing, childcare, food banks, and peer and recovery supports. Keeps contact information, eligibility requirements, and application processes accurate, and adds resources identified by SOC Navigators and other staff.
• Develops a monthly calendar of free and low-cost events in the Jacksonville area.
• Communicates additional service needs to the CSC team and SOC Navigator.
• Provides back-up support for Crisis Stabilization Unit (CSU) discharge linkage when assigned.
• Maintains timely, accurate, and confidential documentation of assistance, applications, referrals, and follow-up in the EHR and designated tracking tools.

Qualifications
• High School diploma and six months of experience working in human services, healthcare, social
services, or a behavioral health setting required.
• Bachelor’s degree in mental health or human services related field preferred.
• Must demonstrate in the RBHS/MHRC Electronic Health Records (EHR) and Patient Information System within three months of employment.
• Proficiency in Microsoft Office, Outlook and use of the Internet required.
• Requires interactions with individuals who may have a mental illness; interactions may take place in an office setting, emergency services, or locked inpatient unit.
• Must exercise judgment required of position.  This includes, but is not limited to, managing work tasks independently, recognizing life threatening situations, safety risks, abuse, neglect, or other emergencies and responding appropriately.
• Must be able to interact appropriately with internal and external customers, including individuals served, their families and support systems, community resource providers, and other department professionals.
