Position Responsibilities
• Complete patient assessments and develop individualized plans of care.
• Coordinate and oversee patient care to ensure quality and continuity of services.
• Monitor patient progress and communicate changes in condition to the appropriate healthcare professionals.
• Conduct scheduled home visits throughout Bloomington and surrounding areas.
• Collaborate with physicians, patients, families, and the care team.
• Maintain accurate, timely clinical documentation in accordance with agency policies and applicable regulations.
• Educate patients and caregivers regarding care plans, medications, and health-related needs.
• Help ensure services are delivered in accordance with established clinical standards and home health requirements.

Qualifications
• Current, unrestricted Registered Nurse (RN) license in Indiana.
• Strong communication, organization, and time-management skills.
• Compassionate, dependable, and patient-focused approach to care.
• Ability to work independently while collaborating with a multidisciplinary team.
• Valid driver’s license and ability to meet company vehicle insurance requirements.
• Home health experience and case management experience preferred.

Experienced RNs are encouraged to apply! We are looking for someone who is motivated, dependable, and committed to providing excellent patient care. Candidates with strong nursing skills and a desire to grow in home health are welcome to express interest.
Schedule and Location

• Position: RN Case Manager
• Setting: In-home patient visits
• Office reporting: Monday–Friday at 8:00 AM to pick up the company vehicle before completing scheduled visits.
