An exciting opportunity has arisen to recruit a part-time Assistant Practitioner to join the Northern Mental Health Social Care Team. In this permanent role, you will support people across North Norfolk and contribute to the Mental Health Duty Team.

You will play a key role in delivering Community-Led Support (CLS) within Adult Mental Health Social Care. You will work alongside individuals, families, communities and partner agencies to promote independence, recovery, wellbeing and social inclusion through relationship-based practice.

You will focus on preventing, reducing and delaying the need for formal care and support by working creatively with people, recognising strengths, building community connections and supporting access to local assets and opportunities. You will develop meaningful relationships with individuals experiencing mental health challenges and support them to achieve outcomes that matter to them.

You will be joining a passionate, forward thinking, supportive and high performing team of Social Workers, Assistant Practitioners, Living Well Officers and Local Area Coordinators. You may be required to undertake Care Act Assessments, Support Planning and Reviews whilst working collaboratively with partners including Norfolk and Suffolk Foundation Trust and various stakeholders.

You will need to have knowledge and a level of experience of the Care Act (2014), Mental Capacity Act (2005) Mental Health Act (1983) and Safeguarding vulnerable adults, as well as excellent time management and IT skills. All new staff are supported to develop a thorough working knowledge of Norfolk County Council processes and procedures.

We operate a hybrid model of working, which includes working from home, our office base at Hellesdon Hospital. There are also opportunities to work from other Norfolk County Council bases as well as other Community spaces.

Adult Social Services in Norfolk is on an ambitious, exciting transformation journey. We're redesigning how we work to create a system that is flexible, human, and focused on better outcomes. As part of this journey, you'll contribute to an environment where learning is continuous, insight is shared, and innovation is encouraged. We support flexible and hybrid working, with a minimum expectation of two days per week spent in a community or office setting, helping you stay connected with colleagues, communities and the people we support.
