What You’ll Do

- Holistic RN Assessments: Conduct in-person clinical assessments, home safety evaluations, and routine reassessments to craft tailored, life-enriching care plans.

- Client & Family Advocacy: Build deep, lasting relationships with clients and their support networks to ensure high satisfaction and long-term quality of life.

- Care Team Leadership & Mentorship: Guide, instruct, and supervise home caregivers to ensure compliance, safety, and top-tier service delivery.

- Quality Assurance & Compliance: Ensure seamless alignment with state regulations, clinical best practices, and overall care excellence.

- Case & Operations Management: Oversee care escalations, navigate changing client needs, and collaborate with interdisciplinary partners and payors.

Responsibilities

What We’re Looking For

- Education & Credentials: Active, unencumbered RN License in state of employment (BSN preferred).

- Experience: 5+ years in healthcare, elder care, home health, or case management preferred.

- Key Skills: Strong clinical judgment, excellent interpersonal and conflict resolution skills, and tech-savviness for timely electronic documentation.

- Requirements: Valid Driver’s License, active auto insurance, and ability to travel locally up to 75% of the time.
