What You'll Do
- Assess social, emotional, and environmental factors affecting patient health and well-being
- Develop and implement individualized social service care plans
- Provide counseling, emotional support, and crisis intervention for patients and families
- Connect patients with community resources, support services, and benefit programs
- Advocate for patients' rights and help navigate healthcare systems
- Facilitate support groups and educational programs
- Participate in interdisciplinary care conferences and collaborate with healthcare teams
- Coordinate services with external agencies and community organizations
- Maintain accurate, timely, and compliant documentation of patient interactions and services
What We're Looking For
✅ Licensed Medical Social Worker in the State of Florida
✅ Strong communication and interpersonal skills
✅ Ability to work independently while collaborating effectively with interdisciplinary teams
✅ Knowledge of medical terminology, healthcare practices, and regulatory requirements
✅ Experience working with diverse patient populations and providing culturally sensitive care
