James River Hospice is actively seeking a compassionate and experienced Hospice RN Weeknight On-Call to join our growing team in Richmond, VA!
In this role, you will provide after-hours hospice nursing care, respond to patient needs, complete assessments and admissions as needed, and coordinate with our interdisciplinary team to ensure patients and families receive exceptional care.
Schedule: Monday - Thursday 5 PM- 8 AM
Salary Range: $85,000 - $105,000

Key Responsibilities
- Respond to after-hours patient calls and provide nursing visits as needed
- Complete initial patient and family assessments to determine hospice needs
- Perform hospice admissions as assigned
- Develop and update individualized plans of care
- Provide pain and symptom management
- Monitor, document, and report changes in patient conditions
- Collaborate with physicians and the interdisciplinary team
- Obtain physician orders as needed
- Coordinate care with LPNs and hospice aides
- Provide compassionate support and education to patients and families
- Complete required clinical documentation
Qualifications
- Active Registered Nurse (RN) license in Virginia
- Minimum of one year of hospice nursing experience required
- Graduate of an accredited nursing program
- Valid driver's license and reliable transportation
- Strong assessment and communication skills
- Ability to work independently during overnight on-call hours
