Baptist Health is looking for a Registered Nurse, Case Manager at Baptist Medical South campus. This is a PRN Weekend shift opportunity working in the In-Patient Case Management Department at Baptist South campus.
Case Manager, Job Responsibilities:
- Care coordination for hospitalized patients.
- High risk screenings and assessments.
- Coordinating post-acute care services with proper documentation.
- Collaborating with multidisciplinary team members.
Experience Requirements:
- 1 year Case Management experience
Education & Credential Requirements:
- Associate's Degree Nursing (ADN)
- Basic Life Support (BLS)
- Driver's Licensed (DMV)
- FL Active licensed Registered Nurse (RN)
