PHYSICAL THERAPIST – HOME HEALTH

Prime Home Health is growing and has immediate openings for Full-Time and PRN Physical Therapists to join our dedicated Home Health team serving San Antonio and surrounding areas.
We are seeking skilled, compassionate, and dependable Physical Therapists , with English/Spanish bilingual ability a plus , who enjoy making a meaningful difference in the lives of patients in the comfort of their own homes.
As a Home Health Physical Therapist, you will provide individualized rehabilitation services designed to improve mobility, strength, balance, safety, and independence while helping patients achieve their personal goals and improve their quality of life.
What You’ll Do
- Evaluate patients and develop individualized physical therapy treatment plans.
- Provide therapeutic exercises, gait and balance training, strengthening, mobility training, and other appropriate interventions.
- Help patients improve functional independence and safely perform daily activities.
- Educate patients and families on exercises, safety, fall prevention, and home programs.
- Monitor patient progress and communicate changes with physicians and the interdisciplinary care team.
- Maintain timely and accurate clinical documentation and electronic medical records.
- Coordinate care with nurses, physicians, OTs, SLPs, and other members of the home health team.
- Provide compassionate, patient-centered care while working independently in the home setting.
Qualifications
- Current Texas Physical Therapist license or eligibility to practice in Texas.
- Minimum 2 years of home health experience preferred.
- Bilingual English/Spanish a plus.
- Bachelor's, Master's, or DPT degree from an accredited Physical Therapy program.
- Current CPR/BLS certification .
- Experience with WellSky/Kinnser, OASIS, and home health documentation preferred.
- Strong clinical, organizational, communication, and interpersonal skills.
- Ability to work independently, manage a daily schedule, and provide excellent patient care.
- Reliable transportation and ability to travel throughout an assigned service area.
