HOW YOU'LL MAKE AN IMPACT
Your Role:
- Performs Minimum Data Set (MDS), case management, utilization review, quality assurance (QA), and discharge planning duties.
- Conducts federally mandated patient assessments based on Centers for Medicare and Medicaid Services (CMS) guidelines, including appropriate care planning, interdisciplinary communication, and Agency for Health Care Administration (AHCA) documentation preparation.
- Promotes wellness.
- Is responsible for collecting integral data and compiling/transmitting data to CMS.
- Determine patients’ needs based upon illness, diagnosis, prognosis, and social support systems.
- Contacts appropriate agencies to coordinate patient discharges into the community or long-term care settings.
- Ensures facility compliance with applicable state and federal regulation.
- Collaborates with other members of the healthcare team.

- Administrator, Rehabilitation Center (JC: 013501) [with delegation (as needed) to RN/Director, Rehabilitation (JC: 030101)]

WHAT YOU’LL NEED TO APPLY
Required Education:
- Associate degree in nursing.
Preferred Education:
- Bachelor’s degree in nursing.
Required Experience:
- One (1) year of relevant work experience.
Preferred Experience:
- Two (2) or more years of relevant work experience
Required Certification/License/Registry:
- Florida RN license.
Preferred Certification/License/Registry
- Minimal Data Set (MDS) certification or ability to obtain certification within one (1) year of employment.
