Social Worker Medical Discharge Planner $5,000 Sign on Bonus

Kline Galland Home is seeking a compassionate and collaborative Social Worker (Discharge Planner) and includes a $5,000 Sign on Bonus to join our experienced and dedicated team on our Transitional Care Unit (TCU) .

Our TCU specializes in short-term, post-hospitalization rehabilitation, helping patients successfully transition from the hospital back to their homes or next level of care. As a key member of our interdisciplinary team, you'll work closely with physicians, nurses, therapists, patients, and families to ensure safe, effective, and patient-centered discharge planning.

This is an excellent opportunity for a social worker who enjoys building relationships, problem-solving, and making a direct impact on patient outcomes during a critical stage of recovery.

The primary role of the social worker is to create a safe discharge plan in coordination with the patient, family, and interdisciplinary team.

- Completion of all required assessments, insurance reviews, and medical chart documentation

- Frequent interdisciplinary communication and meeting coordination

- Caseload management from admission to discharge

- Maintain effective, timely communication with patients and their families

- Understand and meet all government requirements for documentation

Qualifications

- Degree in Social work or related field of study

- 2 years' experience in Human Services or applicable field

- Prior experience in activities a plus

- Ability to manage moving priorities each day

- Strong verbal and written communication skills

- Computer skills for charting and documentation

- Strong team player with the ability to have FUN!
