About this role
We are searching for a Diabetes Educator III. This person provides assessment, goal setting, education and behavioral intervention, and on-going case management to patients and caregivers to promote diabetes self-management in the inpatient and ambulatory settings. Contributes to the development of position papers, guideline development, or other literature to advance the specialty. Develops processes to improve transitions of care and ensure appropriate self-management education and resources. Creates innovative strategies to achieve the Quadruple Aim (population health, quality of patient care experience, costs associated with care, and meaningfulness of the provider's experience).

- Associate's Degree Nursing, Bachelor's Degree or Master's Degree in Nutrition, Public Health, Education, or Nursing requireed
- CDCES -from Certification Board of Diabetes Care and Education Or BC-ADM - Certification Board of Diabetes Care and Education required
- 5 years Diabetes care as a licensed Dietitian or Nurse required
